It is not clear in the quest description but you need to use the flare in your bag when you get to the runestones.

Also note that as of this version, the flare gun may be prematurely removed from your inventory upon turning in Still Beating Heart. If that happens, just abandon this quest and accept it again to get a new gun.

Killing the naga that spawn to defend the runestones count towards the quest Extermination.

Turn off all your addons if you are having trouble using the flare.

How to complete it:
Use the Gob Squad Flare near the Spitelash Runestone, Ticker will appear with the squad and leave a comment. (such as "On it"). Run closer/around the Runestone if he doesnt leave a comment. Defend yourself until Runestone gets blown up. Then head for the next one and summon the Squad again unless they're still with you.

Abandon the quest and get it again if the Flare isnt in your bag.

Quest badly bugged still. Very annoying. Kept dropping and picking it up; followed suggestions above. Cannot progress to next sequence of quests without this one. FINALLY completed quest. Submitted ticket.

When it bugs while Gob Squad are with you I just ran from left to right a couple of yards until they did it. Worked most times but yes still very aggrivating. Took me almost 20 minutes just this one quest.

This quest is kind of annoying but once you get the trick it works:

The obelisks are not clearly pointed in the area on the map, but they have a purple-ish halo around them that makes them quite easy to spot.
You can blast the same obelisk almost right after it exploded, but for me a fresh squad was needed each time to not bug.
First get the area around the obelisks clear to not pull additional trash.
Use the quest item to get a fresh squad. Next thing you need to do is wait for the dialog:

Patch: Ticker, Blow this thing
Ticker: On it.
Ticker: Almost done
Ticker: It's set. Get clear!

Once the last line is told by Ticker, the explosion is almost immediate so get indeed clear.

It takes a bit of time for Ticker to be done, so no worries if it lasts a tad.

Still kinda bugged since it's hard to trigger the goblins to destroy the obelisks even if you stand right next to them. Sometimes it helps to run around them especially the one which is standing on this round stone thing.

Might take a couple of tries to get that dumbass ticker to do anything. Took me 8 flares to complete :-/
Try to avoid the obelisk that's on top of the foundation. Ticker is too stupid to climb to it.

When I couldn't destroy any of the runestones, I realized that Ticker was gone -- killed, I guess. I spoke to Patch and told him "Dismissed." Then I could use the flare gun to call up a new squad. After that I destroyed the stones easily.

Description

Our best demo-man, Ticker, is on assignment with the Gob Squad. He's got WAY more than enough explosives for all the runestones in the ruins.

All you have to do is get him there and keep him covered - he'll handle the rest.
