You have to join a group to Vault of the Wardens instance and find there Cordana Felsong boss (she's the final boss of instance). Defeat her and loot Felsworn Heart. Be sure you obtained it before you left the group, else you'll have to make another full run.

Get knocked out and die just before last boss.
Stupidly release, boss fight started.
After fight receive no loot and get saved to instance.

If anyone doing this with level squish, and not having gear, here's my experience. I noticed with level 51 that you get insane damage buff for legion content. I couldnt solo it with level 50 (i had really bad gear like 50 ilvl), but when i got to 51 level (with the same gear) i could easily solo whole dungeon. Hope it helps.

Description

I have made considerable progress with the Bloodstone, thanks in part to your extensive testing. The stone feeds on blood to grow in power, but its ability to control a demon comes from consuming a corrupted heart; the more powerful, the better. Within the Vault of the Wardens you'll find someone with such a heart.
