It seems as if the icon of this gem has the wrong colour. At the time I'm posting this the icon is yellow, when it's obviously supposed to be red.

This is the WotLK version of the Crimson Spinel, the epic red gem. Already in the game files, it has been officially introduced in patch 3.2.0, and can be obtained in the following ways:

prospected from Titanium Ore;
created by an alchemical transmutation using 1 Scarlet Ruby and 1 Eternal Fire;
sometimes found inside an Icy Prism created by a jewelcrafter;
bought for 20 Emblems of Heroism from Arcanist Ivrenne, Magistrix Lambriesse, or Harold Winston in Dalaran;
exchanged for 10,000 honor points from Captain O'Neal in Stormwind or Lady Palanseer in Orgrimmar.

Cardinal Ruby can be cut in the following variations by a 450 Jewelcrafter (the respective designs can be bought from Timothy Jones in Dalaran, Inessera and Nemiha on the Aldor's Rise and Kirembri Silvermane on the Scryers' Tier in Shattrath for 4 Dalaran's Jewelcrafter's Tokens each):

Bold Cardinal Ruby: +20 Strength;
Delicate Cardinal Ruby: +20 Agility ;
Runed Cardinal Ruby: +23 Spell Power;
Bright Cardinal Ruby: +40 Attack Power;
Subtle Cardinal Ruby: +20 Dodge Rating;
Flashing Cardinal Ruby: +20 Parry Rating;
Fractured Cardinal Ruby: +20 Armor Penetration Rating;
Precise Cardinal Ruby: +20 Expertise Rating;

Update (retail build 10192):

Updated the list of epic design vendors;
Corrected the design price from 5 to 4 Jewelcrafter's Tokens.

Update (build 10072):

The uncut Cardinal Ruby's icon has been updated to match the actual gem color.

Update (build 10048):

Epic gem transmutations now have a 20-hour cooldown.
Cardinal Ruby now costs 20 Emblems of Heroism, up from 15.
Transmute: Cardinal Ruby now requires 1 Scarlet Ruby and 1 Eternal Fire instead of 3 Eyes of Zul.

In Patch 3.2.0 Alchemists will learn 4 of the "basic" gem xmutes and need to complete 5 epic gem transmutations for the quest (which is not yet on the PTR site here on Wowhead) "Cardinal Ruby" given by Linzy Blackbolt. Once you have completed that you will be taught Transmute: Cardinal Ruby. Very simple.

The following gems can be transmuted by an Alchemist without a cooldown. The link is to a blue post confirming what Alchemists on the PTR are experiencing. I have xmuted dozens in a row and the lack of CD is not a bug. So, expect the epic gems to come more into a reasonable line as far as prices go after a few weeks of the patch.

(6/30/09) Build 10048: The 20 hour CD has been placed on epic xmutes and significant changes have been made to the materials required. The information below has been updated as of the stated build.

Its the day after patch day and the gems are selling for over 250 gold easy, everyone wants to upgrade right away so they're selling like hotcakes.

The npc you get the quest to transmute these from is Linzy Blackbolt located in Alch shop in Dalaran. She will teach you how to cut all the gems for gold of course but this one is quested . You must make 5 epic gems and there is a 20hr cool down on these transmute. So it will take almost 5 days or 120 hours for people who got to be specific about it. You do require skill 450 because it takes that skill to transmute and get the quest. Lowest level is lv 65 and she will accept any of the epic 5 epic gems (Ametrine, Eye of Zul, King Amber, Dreadstone, and Majestic Zircon) I don't believe she cares if you duplicate the same transmute or not. Like make Eye of Zul 2x in a row it will still most likely count according to her text. FYI great way to earn gold for Alch peeps out there yeaaa lol. Some of these Epic Gems uncut <3.2 update just came out when I said this> are selling for 300-1.2k on my server. So taking advantage of it while you can! Btw I confirmed most of this with my lv 67 DK and he got 450 in alch.

They are selling on my servers for around 150 gold avarage (Alonsus EU, Alliance).

You can get them through 20 Emblems of Heroism, 10,000 Honor, or if you have the Alchemy skill, then you can transmute using 1 Scarlet Ruby and 1 Eternal Fire.

Farm honor whit wintergrasp weeklies and Battlegrounds. Take these gems from Orgrimmar/ Stormwind and sell them on the auction house. I got 2400 golds in 2 days. i play around 3 hours/day.

Despite the release of Cataclysm, these bad boys are still regularly selling for about 80-90 gold on my server, and the cut versions usually sell for a bit more. If your server-economy is like mine, don't use your leftover Justice Points on stuff like herbs and ore (which cost about 2000 JP per stack which perhaps sell for around 100g) when you can buy these instead, especially if you can cut them.

Right now they are a good thing to spend your Justice Points on rather than on lvl 85 enchanting mats.
On my realm they sell for 120g each, and they cost only 220 JP. Definitely worth selling these instead of 85 things from the JP vendor, this might make you some more money.

Alright, I made another post on this item on another page, so I'm just going to copypasta it here, even though the general idea's already been covered. Perhaps you would like to know a little more information as to why these are so popular.

Here's a great idea for JP dumping, and why to do it - spending the JP's you get on Northrend Epic Gems, bought from Harold, the Alliance quartermaster, or the Horde quartermaster in Dalaran. They are 220 Justice Points a piece.

These prove to be a great moneymaker. On my (rather low population) server, Cardinal Rubies have an almost 1:1 Justice Point - Gold conversion, as they can be cut into gems that give +20 Str, Int, Agi and have no socket level requirement, meaning very low level items such as this relic from Stratholme can accept it. Twinks love these things.

For maximum effectiveness, price gouge! From what I've seen, the primary type of person who buys these epic gems are level 70 twinks, people who have a decked out level 85 main (with deep pockets) looking for a different play style. They pay very high prices for easy to get items such as the Northrend epic gems :)

*Note* Also - if your auction house is flooded with Cardinal Rubies, sell a Majestic Zircon instead! It gets cut into a +30 stamina gem, which pretty much every Blood Death Knight level 70 twink (which is about half of all of the level 70's I see, and the DK never existed back in BC!) throws in every single piece of his gear. This inflates the price of the Solid Majestic Zircon to great heights! Enjoy your exploitation of people that exploit the inherent bonuses of OverPowered classes! You become like a neo-Robin Hood, in a backwards kind of way! :)

The release of Mists of Pandaria is slated for Sept.25, 2012. Every version of the WotLK agility gem is going to be huge because it is the highest gem monks can use prior to the level requirement for Cata gems. This means 精致血石, 精致血玉石, and 精致赤玉石.

As per usual, people who really don't care to spend the gold, or to maximize their toons, will probably be more interested in the uncommon gems because they'll be the cheapest. The players who are sitting on a good amount of gold and are interested in at least maximizing their toons short of spending ludacris amounts of gold to do so will be interested in the rare cuts; and, of course, the players with a vast amount of wealth will pay whatever it takes to have the highest level gems in their gear. That means 赤玉石 and specifically the delicate cut.

These are not the easiest gems to come by and will, at no point, saturate the market. The sheer amount of leveling monks will make these prices skyrocket. Start looking at these on your server's AH for good deals NOW; if you're a JC, go get the 图鉴：精致赤玉石 for 5 Dalaran JC tokens.

In less than 8 weeks, Mists of Pandaria will have the full attention of your server's economy.
