Some facts about Holy Concentration:
+ It procs of any rank of Flash Heal or Heal, from what i could gather the procrate with Flash Heal Rank 1 is just about the same as Flash Heal Rank 7.
+ It can proc of itself indefinetly, with some testing i was able to get around 3 procs in a row, with some luck it can proc a lot of times in a row.
+ While under the effect of Holy Concentration the priest is aparently kept out of the 5 second rule, this coupled with the ability to proc off itself can mean quite a bit of mana regeneration.

- On the downside its relatively unreliable for high end since the procrate is a bit small, and with the change to the contribution of +heal to lower rank spells, spamming Rank 1 Flash Heals is not very efficient unless completly out of mana and on somewhat desperate situations.

This talent also works with Greater Heal spells as well, the biggest issue I have with it is that at times, I have to be careful when the clearcast effect is activated not to hit or use another spell besides Greater Heal (which I usually save it for) or I'll miss the effect completely. Seems great for an extra heal but I need to spend some more time with it.

I've had this for a little while now and it seems pretty clear to me that this is for raid and/or heroics where you are desparatly spamming heals over and over. With 5-mans I do direct casted heals so rarely that even if it procs and even if I happen to notice the proc... I have no one to cast it on. I might see it proc once in a whole dungeon run b/c I'm using mostly PoM and Renews.

Have been raiding now for several months and to be honest I don't even pay attention to this. I am one of the CoH aka raid healers and throwing very few greater heals. Though I have had talent points in this for some time I am actually dropping it for Prayer of Healing and Holy Specialization. If you find your self a tank healer this would be something to have.
