To upgrade your Heart of Azeroth, you just need to do the 8.2 War Campaign starting with Horde: The Warchief's Order The Warchief's Order or Alliance: The Wolf's Offensive The Wolf's Offensive until you get to this quest.

At the end of this quest after I receive the Dreamglow Dragonscale it tells me to take the portal from the Crumbled Palace to Stormwind, but I can't find the portal...

After finishing this quest can one return to the "Emerald Dream Zone" again by talking to Merithrah in Temple of Elune in Val'sharah just to visit; or does completing the quest remove Merithra and/or the option to enter the "Emerald Dream" zone?

I completed the quest. Unfortunately, when I return to the Chamber of Heart the quest giver, Merithra of the Dream, is nowhere to be seen. There is a '?' showing on the outside of the CoH, not the inside. Merithra is nowhere to be found. Is there something else that must be completed?

To turn this quest in, go to Silithus either by flying or taking the portal from Zuldazar. Talk to the lady at 78, 22 to enter the alternate timeline, the one where Silithus is on fire. Then fly to Magni's camp at 43, 44 and use the portal to enter the Chamber of Heart.

Return to Merithra in the Chamber of Heart with the Dreamglow Dragonscale.

	Speak to Merithra to leave the Emerald Dream (1)

Description

There is nothing more you can do for us here, champion. You have the power you sought, and the Emerald Dream is fighting back against the Void. Bring this dragonscale back to Magni and the others. I will meet you in the Chamber of Heart to help infuse it into the Heart of Azeroth. As for my flight, our long vigil must continue... or we will be lost to the Void.
