If you take too long in the vision (we're talking afk'ing for 15 minutes or more) to complete this quest, Lady Naz'jar will eventually be slain by the Kvaldir and leave you unable to turn in. To fix this problem, leave the vision and wander slightly away from the quest area before re-attuning yourself to the battlemaiden.
