It looks like if you have the quest, A World Worth Saving, then it will bug out. I just dropped that quest and it worked fine after going back in.

I ran around like an idiot while my bar went down, tried to engage the elite in the courtyard, saw that wasn't going to work, ran back to the spot where I started, and it said that I had "calmed the whispers" but then I immediately died, and when I ressed, I was back in the Chamber of the Heart with Magni and I was able to turn in the quest.

If the quest bugs for you, in my case Magni did not move at all. Abandon the quest and retake just outside the entrance.

Okay, I had to do this quest four times before I worked out how to do it.

When you enter the Halls of Origination you'll be in the entrance passageway. Take a step forward and it'll start collapsing; there's some talking and you're transported out into Ptah's tomb. Walk outside and there's some whirlwinds and an alter to N'Zoth. When you approach the alter it'll push you backwards. When you click on the alter you'll enter a tiny vision of N'Zoth

Now, what I was doing was running straight over to the alter and clicking on it. Because I was a death knight I used Death's Advance and its pushback didn't slow me down. Don't do that. The vision will activate but you won't hear Wrathion. Killing the mobs doesn't help. Trust me. I spent five minutes kiting the elite to death.

Wait for Magni. After you appear in the tomb he runs over to a teleporter and activates it for some reason. Then he slowly runs across the courtyard. Then he very slowly walks up to the alter. Wait for him to get there. He should have three voice lines, and until I waited for him I was only hearing the first.

Magni Bronzebeard says: Look! Across the sand! That must be the anomaly!
Magni Bronzebeard says: Wait... do ye feel that? Somethin's wrong...
Magni Bronzebeard says: Be careful, hero!

You'll know it's worked if Wrathion starts talking once the vision activates. From other comments on this page, you don't actually have to dismiss the whispers to complete the quest. Dying from sanity loss will work too. If it bugs out you can just abandon the quest. You'll be transported outside and Magni will be waiting to give it back to you.

Seems to be more than one quest that can trigger the bug. I had to abandon like 10 quests from my log, resetting this quest a bunch too before it would trigger. If you can get Magni to do anything, play around with dropping quests if dropping this quest alone does not work. What a pain.

I've done this on my main and 3 alts now (fortunately it has never bugged out for me) and I hope this information is helpful. I would have added this to my reply to Zincite's comment except "replies" are character limited :(

TL;DR:
After "Magni Bronzebeard says: Look! Across the sand! That must be the anomaly!" You can start across the sand. As soon as you interact with anomaly and get ported into vision interact with the 5 "whispering thoughts" mobs right around anomaly and wait for Mother to port you out. Don't waste time trying to kill stuff.

- The first video on this page should be ignored. It is from the PTR and things have changed in the dialog and actions to take.

- The second video is accurate but when Character gets inside the anomaly he starts killing things first. He then figures out to interact with the "whispering thoughts" mobs right around anomaly. But, before he can interact with all 5 he dies of Sanity loss. I did the almost same thing my first time through ;) Although I did interact with all 5 I lost all sanity before Mother could complete my extraction.

- As Zincite points out elsewhere you can die of sanity loss and still get quest credit (yay!). However, when I died on my main I suffered the 10% durability loss (Boo!). (Not a biggy, but that runs about 70Gs in repair cost for me nowadays). Please note this was on day 2 (Jan 16th?) of patch and maybe been updated to eliminate the 10% durability loss.

- The other 3 times I've been through I immediately interact with the 5 "whispering thoughts" around anomaly and have gotten ported out with about half sanity left.

Don't waste any food or buff cooldowns prior to doing this quest, odds are you WILL die from sanity loss. Quest still completes and you'll find yourself back in the chamber, but you won't waste anything.

Magni is kinda bugged, sometimes staying at the entrance. Surprisingly people advised to abandon the quest to give it another try, well I just disconnected / reconnected standing close to him and it was fine, no need to rush back to the entrance or anything :)

If the quest bugs on you like it did me today, just do a file check; it usually takes 5-30 minutes. I worked for me, and fixed the bugs.

BUG: If Magni just sits at the entrance and doesn't move try to log back out and back in. This corrected it for me.
