You have to choose Altruis the Sufferer or Kayn Sunfury.

The one you choose will end up as your Order Hall follower:
Champion: Altruis the Sufferer - Altruis the Sufferer
Champion: Kayn Sunfury - Kayn Sunfury

Also if you choose Altruis the Sufferer you'll get Akama champion. Kayn Sunfury - Shade of Akama

A New Direction is part of the Demon Hunter starting quest chain. - Vault of the Wardens storyline.
We were chained in Vault of the Wardens after the events of previous Mardum quest chain. Now we are in Legion time and the Wardens prison is under attack... We have to escape and save Illidan, but Gul'dan manages to take Illidan cage with him.

I'm not sure if this quest is currently bugged. I chose Altruis, completed the quest, but did not receive the helm. My chat log shows the quest completed, gold received, and the Helm of Reaffirmed Purpose has been added to my appearance collection; it does not show that I received the helm and it is not in my bags.

I chose the "traitor" and ended up in Stormwind as Alliance XD

I tested this by doing a blood elf and a nightelf. I picked the traitor for both races.

It does not matter who you choose, blood elf will still go to the Horde and the nightelf will still go to the Alliance.

In A New Direction you speak a Kor'vas Bloodthorn you have a choice between Kayn Sunfury and Altruis the Sufferer.

This story choice will not
affect your character's power.

Altruis the Sufferer

Altruis the Sufferer is a
renegade who takes matters
intro his own hands.

Although he and Illidan share
the same goals, he often
disagrees with Illidan's
methods.

Believing Illidan no longer
controls his demonic powers,
Altruis wants new leadership
for the Illidari.

This story choice will not
affect your character's power.

Click the Pool of Judgment to be given a description of the choices, Kayn vs. Altruis. Devixon has already transcribed the text you'll see from the pool here.

From what I can tell, the decision here is purely a roleplaying one because Kayn Sunfury and Altruis the Sufferer have identical abilities and traits.

It would appear that this choice determines an additional follower later (per varenne (here)) - also between followers with identical abilities and traits.

If you click on Altruis the Sufferer or Kayn Sunfury a few times they will say this (one line for each successive click):

Did you just touch me?
I wouldn't if I were you.
Normally I hunt demons, but for you I'll make an exception.
Do it again and your skull will be my wetstone!
You're no demon, but I'll put you in the ground just the same!
I WILL FEAST ON YOUR SOUL!

If you're trying to remember who Altruis the Sufferer is, he's the demon hunter with a nether drake in Outland's Nagrand, and he has you beat up a retired demon in Shattrath so you can learn how to take out the nearby forge camps.

I really did not want to choose Altrius because i felt more of a bond with Kayn during the questing but seeing how i chosen Kayn on my other DH i had to see how Altrius would play out even though i had no connection to him or even wanted to be associated with him!.

Anyone else feel that way?

1. A New Direction
• Accept the quest, A New Direction, from Kor'vas Bloodthorn at coords 24.4, 55.9 (The Warden's Court of Vault of the Wardens).
• Turn to the Pool Of Judgement (coords: 24.5, 51.5), a "pool" at the center of the room and click on it to watch the Illidan Harbingers cinematic.
• Speak with Kor'vas Bloodthorn at 24.4, 55.9 and select if you will follow either Kayn Sunfury or Altruis the Sufferer.
• Turn in the quest to Kor'vas Bloodthorn.

Be aware that if you make a different choice from the other Demon Hunters in your group you will be put into different phases for the Bastillax fight for Between us and Freedom. You'll be put back into the same phase after you kill Bastillax and exit the vault (before you turn in the quest).

If you have any regrets in between making your selection and handing in the quest, you can abandon and re-accept it from Kor'vas and make a different decision.

After reading all the posts, I'm going to choose in the following way:
If I already had an odd number of DH-s (1, 3, etc.), then I would choose the one whom I have chosen less times. But this is my first DH, so I really have to choose.
Akama at least seems to have a face (opposite to Shade of Akama), and I don't like faceless NPC-s. That's why I'm going to choose Altruis the Sufferer.

Description

<name>, as leader of the Illidari in Lord Illidan's absence, you must choose your second-in-command. Kayn or Altruis. You can only choose one. The chosen shall become your personal champion, and their ideology will color our decisions for the Illidari going forward. No matter who is chosen, our mission remains unchanged. If you need a reminder of our past, gaze into the Pool of Judgment. Remember the sacrifices that forged us into demon hunters. Speak to me when you are ready to choose.
