The three questgivers for tier gear are located in an alcove beyond The Prophet Skeram. They are only accessible once this boss has been killed.
Each quest will require a a combination of rare-quality idols and uncommon-quality scarabs. These combinations vary based on your class and item.

Reputation:
Brood of Nozdormu reputation can be earned through killing bosses in both Ahn'Qiraj instances, killing monsters inside the Temple of Ahn'Qiraj, and doing quests related to the dungeons.
Players start off at 0/36000 Hated. Getting out of Hated is the longest part of the reputation grind.
Killing trash maxes out at 2999/3000 Neutral.
Qiraji Lord's Insignia
drop from every boss. Turning them in for
Mortal Champions
, a repeatable quest, is 500 reputation each.
Completing a quest for tier gear also grants 500 reputation.
To maximize reputation, save all insignias until capping reputation via trash.
C'thun alone grants 2500 reputation.
Ancient Qiraji Artifact
, a rare drop from trash, begins

which grants 1000 reputation.

Tier 3 gear was originally obtained by raiding original Naxxramas. Every item required additional zone-wide drops to convert into wearable armor. This is the only 9-piece set.
In Mists of Pandaria, this gear was put on the Black Market Auction House, located in

. In Warlords of Draenor, this has relocated to the Ring of Blood in

The items found on the BMAH are randomly generated by the server. Bidding starts at and can go up to 1 million.
Class restrictions weren't added to these armor sets, so all classes that use a particular armor type can transmogrify to these sets.

Scholomance sets are no longer available due to Scholomance's revamp in Mists of Pandaria. They didn't totally match in the first place, but held sentimental value.

is no longer available due to Deadmine's revamp in Cataclysm, although boots, pants, and gloves can be gotten through other means.

Garrison Sets
Your Garrison offers many unique sets tied to different buildings.

Warlords of Draenor lets you craft some epic items even if you do not have that profession. You can do so by leveling profession buildings to level 3.
To initially build the profession building, you’ll either have to have that profession and loot a world-drop item that starts a quest for the building, or hit Tier 2. At Tier 2, you can purchase all Tier 1 blueprints.
To upgrade to level 2, you’ll need to buy the blueprint after completing Talador or hitting 96. To upgrade to level 3, you’ll have to be 100 and have completed

If you do have a profession, you can purchase items to upgrade your crafted gear to ilvl 655 and then ilvl 655. This changes the color of the armor.

Warfront Normal Set:
Armor pieces of the most basic appearance of the Warfront tint are obtained randomly after winning The Battle for Stromgarde Warfront. This reward is automatic and shown on the victory screen of the Warfront and can be obtained multiple times within the same week. You can also farm Darkshore Rares once per cycle for these armor pieces.
Warfront Mythic Set:
Armor pieces using the most elaborate appearance of the Warfront tint are obtained randomly from the Warfronts Equipment Cache, a reward given from the quest, Warfront: The Battle for Stromgarde, for winning the Battle for Stromgarde Warfront scenario for the first time during a cycle. You can also obtain one or two pieces from the Darkshore World Boss,
