Sounds really interesting and challenging. Wonder exactly what raid lockout entails. Will the same 10 have to return or will you be able to come back with some different folks? I'm guessing if you can complete the dungeon without dieing though you can clear it fast enough that you won't be leaving and coming back another day though.
Anyway, looking forward to the challenge of getting this title.

Heroic mode on any dungeon is when you complete the fight the hardest way possible IE: leaving all three drakes up on the WotLK onyixa(slipped my mind) or doing the Twins in the harder manner.
you may be correct that it is only do-able on 25 man versions but idk that exactly.

Given that Naxx is the first raid instance in Northrend, it's quite possible that it will end up being what Kara is to BC - somewhat difficult when the expansion first comes out, but after a few months a walk in the park. Yes, I know it wasn't a walk in the park in vanilla WoW, but the Northrend version is going to be different.
In any case, I like to think of a lot of these 'special' raiding achievement (do a boss with less numbers, with a special feature, no-one dying etc.) as something similar to the Amani War Bear - when you first start raiding the place you'll have next to no chance of getting it, so you'll just focus on progression. But eventually you'll get so geared and so proficient at the fights that you'll be able to start pushing the boundaries and going for the challenge.
In any case, I'm desperately hoping I'll be able to get the title!

Il was wondering about one point. Is the achievement lost for the lockout if someone happens to die during an encounter, or only if a boss is killed when someone has died during the current pull ? ie can we reset the boss if someone gets killed and pull it again to get credit for the achievement ?

), dying during a boss pull triggers achievement failure. It would thus be impossible to make the raid wipe and go again.

To complete this achievement, no player in the raid is allowed to die during any boss encounter. Trash deaths (even wipes) are fine though. It is possible to swap players in and out of the raid. Of course, only players who participated in every bossfight are eligible for this achievement.
As of today, it appears to be that only two guilds were able to complete this achievement:

with the Druid Bug.. it's just too easy.. we bring only 2 druids + 1 shaman as healer
and requesting all members to have 20k buffed HP.
we cleared whole 25Naxx in 4hrs on the same night :/

While it's obvious to many, I thought I'd provide a list of the bosses and their most dangerous aspects.
Noth:
pretty easy fight. DPS has to be careful of teleports; tanks have to be on the ball with picking up adds or they'll aggro a healer.
Heigan:
very obvious. Dance well. Aside from this, a person or two have to be quick on disease cleansing.
Loatheb:
nothing major. Don't pull aggro, and assign the healing roles beforehand.
Anub'rekhan:
nothing too bad. Healers need to make sure they aren't silenced during locust swarm.
Faerlina:
pretty simple. Move out of the fire. If you're not going for the achievement, assign a healer to cleanse the poison bolt volley. If you are going for it, use something like a dps shammy or boomkin to cleanse the groups without Poison Cleansing totem, as your healers will need to be putting out high hps on the Faerlina tank.
Maexxna:
if your guild's dps isn't too high, make sure you don't burn Maex under 30% until just after a web wrap. If your dps is high enough you don't really need to worry. Save damage reduction cooldowns (Pain Suppresion, Guardian Spirit, Hand of Sacrifice) for any enrage Web Wraps.
Razuvious:
the only fatal parts of this fight are Jagged Knife and MC drops. Jagged Knife simply has to be watched by one dedicated healer. To prevent MC drops as much as possible, have your priests drop MC after each round of tanking, and then re-MC the add (or MC another one depending on HP).
Gothik:
nothing too bad. Tanks need to be on the ball for picking up the death knights and riders.
Four Horsemen:
this is one of the dangerous ones that doesn't involve instagibbing like Heigan or Thaddius. There are two main obstacles to overcome to ensure success on this fight (in the context of the achievement). The first is Thane's meteor - on the Thane tanking side nobody can be on their own. Meteor needs to be shared with at least 2 people in order to prevent an instant kill. It sounds easy enough, but it's quite possible for that healer or hunter to stand back just far enough and get killed by being on their own. Aside from Thane, Zeliek is the real killer. Due to his holy bolt (the chained one), everyone needs to be spread out when dpsing him - which can be difficult on the 25 man version. The easiest way to prevent dps dying on him? Have your melee dps do nothing on him, and stand far away from him. It's slower, but safer. In fact, you can just have a few ranged dps on him and have everyone else go sing around a campfire. There's no penalty for taking it slowly to ensure no-one dies.
Patchwerk:
pretty straightforward. If you're using one off-tank, make sure melee monitors their health and takes a slime bath if necessary. If you're using two off-tanks (much safer), make sure they're constantly healed.
Grobbulus:
provide enough room for the diseased people to explode. If anyone DC's, pray. Make sure only one person is assigned to cleansing, and that they are using only a single target, single tick cleanse (ie. Abolish Disease is devastatingly bad).
Gluth:
as tank healing is pretty easy, the focus becomes the kiting. Kiters need to ensure they're getting hit as little as possible. Healers need to be spamming them with big heals if they ever need to move close enough to the zombies that they do take a few hits. They need to be healed up ASAP after a Decimate - the same goes for the Gluth tanks, especially the one currently with aggro.
Thaddius:
another instagibbing boss who is quite unforgiving of lag and slow movement. There's not much to say about him - make sure everyone in the raid knows the strat, and remember anyone that fails to execute it for next time. An important note - if you miss the jump and fall in the slime, you can't immediately move into the positive or negative group (once you go back and successfully make the jump) as you'll die due to lack of a charge. You need to stand off to the side of Thadd (about 20-30 yards, but not close enough to the current groups to cause problems) wait until Thadd casts his next polarity shift, and then join the group appropriate to the charge you gain. Easiest way to get around it? Don't miss the jump.
Sapphiron:
the third instagibbing boss. The fight itself is pretty straightforward, but make sure you have a health potion and healthstone available to play on the safe side. Ensure everyone knows how to get behind ice blocks - and specifically, not to get behind ice blocks which are too close to his inner circle, as they don't actually block LOS. If you're still on track for the Immortal at this point, it's highly advisable to arrange for your healers to try and move to specific ice blocks, so there's at least one on each. While having no healer at an ice block isn't necessary fatal, it makes things very dangerous if there's a blizzard on the ice block and no-one to heal up the damage.
Kel'thuzad:
there's actually not too many dangerous factors in this fight. In the adds phase, as obvious as it is, don't peel the aboms off the tanks (and tanks be on the ball with picking them up), and melee don't get anywhere near the banshees. Make sure skellies are killed before reaching the raid. In the Kel'thuzad phase, make sure you move out of void zones, and spread out - using the range function on Deadly Boss Mods (can be opened by /range in the current version) is quite handy. Melee needs to setup their triangle/square formation before the fight, and if they need to move away from a void zone they must not move within range of the tank. Assign a strong healer to each of the add tanks. Frost Blast is the major killer in this fight, but it's actually not too bad if you plan appropriately for it. Firstly, your healers need to be spread around the room. The tank should be fairly centralised and all healers should have range on him. When a Frost Blast occurs - any healers within range need to heal it. As the tank will be in range of everyone, it's safe for a healer to stop healing him for a moment (and it's incredibly unlikely he'd die within 2 secs on KT even if all healers stopped on him). Frost Blast does 104% damage over 4 secs, so as long as you get one heal off the person should survive. However, multiple Frost Blast victims is a little trickier. Ideally you won't have it happen, but of course it sometimes does. To combat it - heal on person, then the next as quickly as you can. For me as a healadin, I would Divine Favor + Holy Shock the first, healing them, then I would instant Flash of Light the second (thanks to the Holy Shock crit) healing them. If you get 3 or more blasted, do your best, but it's very difficult to keep them up unless you're lucky enough to have all the healers in range target a different victim first. Besides that, stay spread out, avoid void zones, and keep the tanks and raid up.
As you can see, there are a lot of things that can kill a single raid member. It's not an overly hard achievement, but it requires a lot of things to fall your way. It's kind of like Tetris - in essence it's fairly simple, but in reality, if you get a string of bad blocks there may be no way to make a decent game of it.
That covers most of it I feel, but if you can think of any other parts of boss fights which are prone to easily killing a raid member (or a few), please reply and keep the list growing. ^_^

I can safely, and completly honest say, that Frogger event after Patchwerk,
DOESNT COUNT
to the achievement. (Undying or Immortal) Yesterday we had 1 death there, and we still got achievement.

Achieved The Immortal title yesterday, so I figured I'd share some tactic adjustments and just random thoughts during the battle in general and from healers perspective in particular.
We went to this event with 6 healers, 4 tanks and 15 dps, but due to RL issues 1 tank and 1 dps had to drop out, so 6 healers, 3 tanks and 14 dps by KT.
Razuvous - we started off with him , because this fight has the most RNG from the 1st bosses and if you fail you can do a soft reset (everyone out of Nax/raid and a new RL make it. One thing to note: do not have shadow resist aura and/or frost aura. If you have a dk, specced into frost aura, he will have to either respec or hit the boss once and run away a bit to range out the aura effect on MC targets.
Pretty straightfoward for a healer. If you're on raid, just watch for who Razu targets and precast heals on him to avoid any chances of chaining damage.
Gothik - easy, just stand in the corner and run after tanks if things go rough.
4HM - Make sure everyone stacks on eachother on Thane. Burned down Thrane with all dps on him + hero (didn't even reach 4 stacks). Then same for Rivendale. In the back people switched on 2 marks to avoid any chances of spike damage. We also took it the slow way and made most of the melee swap/stay out on Zeliak to avoid any chances of huge spikes from chain holy wrath.
Noth - we had a tank stand in each of the adds spawn corners, picking them up immediately and all casters in far left corner of the room to avoid any chances of clothie getting hit.
Heigan - pretty easy as long as people know the dance. We had 3 people assigned specifically to cleanse diseases.
Loath - nothing to wipe from.
Patch - we had our melee stand away from the boss for around 10 seconds to avoid any chances of somebody popping in the #2-3 threat range. After that it's tank'n spank.
Grobb - We had 2 healers assigned to running after injected people; Grobb was also tanked a bit further away from the wall so we always had a spot to run in with the injection.
Gluth - 2 tanks kiting, healers chaining cds on tanks for decimate (should only hit once).
Thaddius - no problems here as long as people know how to switch sides. We also made it clear that nobody enters the fight if he fails the jump (didn't happen, but being extra safe doesn't hurt)
Anub - no problems.
Faerlina - we killed all adds to avoid spike damage and just healed off the enrage with 4 healers assigned just for MT.
Maexxna - chained cds for web spray, no problems after that.
Sapphiron - we did it on 1 side, but had healers spread out on it. 2 healers closer to entrance to KT room, 2 healers to center and 2 healers to entrance to Saph. To ensure that each side always has at least 1 healer for breath time to top everybody off.
KT - staying calm is a must. Healers were spread out in a similar fashion as on Saph to ensure that all raid is in range of healing for FB, including healers themselves. We had few people specifically assigned to CC the MCed. If you use DBM addon, then use /range to avoid any chances of getting FB chain. Again, staying calm is a must. You must understand that RNG may and most likely will play against you, so be prepared. For example, we had our OTs with adds on them FBed, healers FBed, our main tank MCed. As long as people are calm, it'll stay under control.
On a general note we also had healers ready to chain CDs on dps that have hard time controlling their threat/running away from stuff that hurts. Saying "no he stood in x, it's his fault" won't bring you your title.
As a DPS please remember that if you're attempting this, then you are most likely already well geared and DPS races should not concern you. Staying alive is a priority. Run away from Blizzards, void zones and so on even if they hit you for little damage.

Presumably Blizzard will want to keep this achievement hard so the requirement may be changed to Ulduar/whatever else they invent/Icecrown. This isn't confirmed or anything, it's just a likely thing they'll do.

Lost this achievement last night when KT was around 20%. If you are going for this achievement it might help to have melee take their AOE abilities off of their toolbars. We had a member get MC'd then killed when all of our Melee decided to use their aoe around the same time. =( so close.

As the criteria explains, you only need to be present for the downing of KT to get the title. No raid member must die on any of the bosses for that night/week (because this can be done over then span of the week before the raids reset).
My guild Occult of Jaedenar was the 65th US guild to get this achievement on March 18th, 2009.
Now I am sporting the lovely Naiyia the Immortal.
This achievement is by far the hardest in game due to the difficulties with keeping everyone alive on each boss encounter and having nobody disconnect.
However in our run a rogue dcd on Four Horseman and the MT got blinded on KT. BUT we still got it! :D

In care anyone is wondering - Getting aggro on Kel'Thuzad at 28%, getting oneshotted and be the first person to die during the run WILL make your guild hate you for at least 1 week.

Got the achievment last night, finishing off our Heroic:Glory of the Raider. Black Proto Drakes, ftw! Server first.
Anyway, here's a trick to note. Start the wing where the first boss is most likely to kill one of your raiders first. That's usually Patchwerk nailing a melee, unless your Shadow Priests are really bad at MC on Raz.
Then, if you take a death before killing the first boss, wipe the raid. Disband, and reform under a new leader. Since you weren't saved to the old Raid ID, reforming the raid gives you a new one.
This way, if you get unlucky, you've got a second chance at Immortal without waiting til next week. Won't help you if you take a death after you kill a boss and get saved, but Patchwerk can be a pain, even for the best of us.
This is tested and confirmed to work, even though a GM told me during the raid that it wouldn't.

If you make it to Kel'thuzad there is but ONE option to succeed in this achievement.
Interrupt. Every. Frostbolt.
An enhancement Shaman with a bit of a tuned spec can take care of 75% of the interrupts, but you don't want to rely on just that. Our guild has a rogue rotation with the enhancement shaman kicking as many as he can, and anyone else with an interrupt spell attempts to interrupt every frostbolt they see. Even I, as a warlock, use my felhunter and have a spell lock macro ready, and I can manage to get 8-10 interrupts per fight, without hurting my DPS at all since it is a pet ability and thus does not affect my GCD.
Doing this ensures that when the mind control happens a random DPS does not take a 30k frostbolt and die.
Also, make sure to have all of your DPS keeping their damage increasing cooldowns on cooldown, to prevent the MC popping them. You don't want a warrior popping deathwish and two-shotting a clothie. CC is also extremely important; sheeping, cycloning, fearing, rooting, and anything else that you can possibly use to keep the MC targets under control should be used.
Taking an extra tank is also very viable, having 2-3 off-tanks to make sure the spawns are picked up right away is essential. If you keep this all under control, The Immortal is as good as yours.
Oh, and don't stand in a void zone and die, particularly at 4% on what would be your 2nd time getting the achievement for the rest of your guild that missed the first one (/glares at a certain death-knight offtank)

Also, a nice thing to note, if Baron Rivendare is dead and thus is not the result of a death on Four Horseman, but someone still dies, the achievement can still be obtained. Baron counts as the "boss" for this fight, so once he dies any other death does not count against The Immortal.

I'm not sure I believe this to be true. As previously stated, it's commonly accepted/thought to be true that you need to be successfully OUT of combat. If you down Riven, but die on the subsequent mini-bosses, you're still in combat and this would negate the achievement.
This is why you never pull slimes immediately after Heigen, before you go OUT of combat.

My guild finally achieved this tonight and figured I'd share something interesting that happened during the run. We actually had 2 deaths on the 4H encounter. We killed Thane first and immediately afterwards we had a death, note here
Rivendare was still alive
. We had another death while killing the last horsemen. We had thought we failed but had no other deaths during the run and everyone got the Immortal after killing KT.
So that either means that deaths don't count after the first horsemen dies, regardless of which one, or that the 4H encounter doesn't count at all towards this achievement. I'm willing to bet on that as there a rumor it's counted as an "event" and not a boss, ie the bosses don't drop loot but when all dead trigger the chest.
Looking at the Dedicated Few achievement the 4H don't count there either. Would be interesting to see if a guild could cause a death before any of the horsemen were killed and then keep trying and see if they still got the achievement.

As of 3.3, killing Sapphiron and Kel'Thusad before clearing the other wings prevents The Immortal or Undying achievement from triggering.

My raid group just completed this achievement and, strangely enough, we did not have to clear the whole instance.
The order we used was Razuvious -> Sapphiron and Kel'thuzad -> abomination wing -> plague wing -> Gothik and the 4 Horsemen. After killing the 4 Horsemen, we all got our Immortal achievement, despite not having killed any bosses in the spider wing.
I don't know if this means that the spider wing is no longer required for the achievement, or even that you can get the achievement by just killing the 4 Horsemen. I just thought it might be worth pointing out in case someone wants to try this (although if the former is true, then that's not much help, since the spider wing is unlikely to be a problem if you can clear the other wings with no deaths).

Quick question because I'm getting so much mixed posts.
According to someone, a person can join a clean raid that's at KT and get the achievement.
Does this mean that you only need to down KT without any raid deaths to get the achievement?
If that's the case, couldn't we just teleport to frostwyrm lair and go through sapph then KT?

We did it, we completed it no deaths. Buuut no achievement. After 3 days of waiting for the ticket the GM pretty much said "sorry for the confusion, but the achievement is completely taken out of the game." He also gave us the usual "Post on the suggestion forums."

I remember seeing people sell this achievement on our realm. They charged 10.000 Gold per player. Decided to tag along on my alt since I wanted the title but going through the whole recruit mess again was not an option, so I bought it.
It was rather interesting how they executed it,
1. I payed a level 1 char in Dalaran 10.000 Gold (Risky I know, but there was 15 people paying him)
2. I got invited by a stranger.
3. I got a summon.
4. I'm standing in front of
