This is the only lvl 1 headslot item far as i know.
Except for the santa hats.

EDIT: This is not true. There are other items such as Belbi's Eyesight Enhancing Romance Goggles also from brewfest, and new ones in WoTLK: Violet Hat and Purple Turban

This Hat is used in Disturbing the Peace, one of the Brewfest Achievements.
In order to get the Achievement you need three parts of the Brewfest cloth, thats:

One of the Brewfest Hats either the Blue Brewfest Hat, the Green Brewfest Hat, the Brown Brewfest Hat or the Purple Brewfest Hat
Either the Brewfest Dress or the Brewfest Regalia
And finely the Brewfest Slippers or the Brewfest Boots

There is only one thing to say about this amazing hat, OKTOBERFEST!
In all truth, the hat is actually a tyrolean, a type of hat worn in the Austrian Alps and Bavaria. The tyrolean is a hat commonly associated with the German celebration Oktoberfest and Swiss mountain climbers. The hat itself is named after the Austrian state it originates from. The state, Tyrolean was ceded to Italy after the first World War.
