I've found a couple of them. Listing what I've found here so people can build off it. I didn't use coordinates so this is just geographic guesses.

1-2) No clue yet

3) Mushroom Cap - On the east side of the mushroom forest on a pillar there's a guy U'llort the Self Exiled who wants a cap "Story long have you boskroot cap". You must find a boskroot cap in the forest (i found it on the southwest edge). On giving the cap to him, he gives you a mushroom treasure.

4) Mosswoll Flower - North of Rambleshire, there's a lost sheep that you click on to help back. Doing this will make it run away. Follow it and click on it again. It runs away, then click on it again. This time it drops something. Click on that thing. Gives a pet.

5) Jade Pearl - Found on the coast line of Three Shields.

6) No clue yet

7) Turtle's Thanks - There's a Dalaran Sewer Turtle who asks for dornish pike near Freywold. The pike can be fished up by fishing at calm surfacing ripple. After giving it to him, it needs a little time (about fiveish minutes), and then asks for a goldengill trout. These are fished at glimmerpools. After giving him the fish he "needs time". After a couple minutes, he can be found in Dornagol with his 3 other DEFINITELY NOT NINJA TURTLE friends and gives you a thank you gift. It's a Dalaran Sewer Turtle Pet.

9) Thak's Treasure - In Southern Storm's Watch, there is a guy named Thak who asks to play a question game. Ask him the prompted questions and then he leads you to a rock. Click on that rock to recieve Thak's Treasure.

10) Kobold Pickaxe - I found it, but didn't write down where. Should be fairly easy to find though.

11) Opal Lily - At the base of the Opalcreg big mine cave around the pond to the far right. It despawns when someone clicks on it so it may take a bit to respawn.

12) Defender's Axe - Found in a massive towere at the top of a cliff in "far NORTH Of Rambleshire". Just click on it.

Other things I found:

One of the prizes comes from an Angler Fish named Llionel on the coast of Donagol by a massive rock. After kicking him, he demands a meal. No clue what that meal is yet, as nothing I cooked worked. Maybe something from Herbalism?

There's also something about Pearlescent Crabs in a cave on the largest mountain (the one north of Dornagol). You get a crab luring stick to convince a pearlescent crab to follow you, don't know where to find a pearlescent crab though (that isn't the one already on the giant).

Sapphire Crab Pet (& Lionel Location) Guide - https://youtu.be/xGtkQT688ZU

1. Head to the small island Southwest of Dornogal and find Lionel under a rock pillar on the North tip of the island.
2. Speak to him and kick him in to the water, he will then ask for food.
3. Search the shoreline of this same island for Plump Snapcrabs, they are small and blend in with the sand kinda.
*Tip: Use the foillowing command to target crabs: /tar plump snapcrab
**This will make it much easier to find them.
4. Collect 5 Plump Snapcrab and return to Lionel.
5. Feed Lionel and he will leave behind a treasure chest with the Sapphire Crab pet inside.

Lil Moss Rosy Pet Guide - https://youtu.be/Ct1AvR94X-0

1. Locate "Lost Mosswool" on the edge of a hillside North of Rambleshire.
2. When you click on her, she will run a short distance to the South.
3. Follow her and click again, she will run down a little bit further South and will stand in the middle of town near the sheep pen.
4. Click her a third time and she will leave behind a lootable item: Lil Moss Rosy.

Hanna's Locket / Buttercup Hologram Guide - https://youtu.be/iwbVGN3FxzI

1. Visit Freysworn Letitia inside a cave behind the waterfall North of Dornogal.
2. Talk to Letitia to receive a Crab Guiding Branch.
3. Visit 6 locations to collect Pearlescent Shellcrab

Hint: copy and past the following command: /tar pearlescent shellcrab
*Enter that command in the chat box in order to targer a shellcrab when you are close enough, this will make it easier to find them.

Shellcrab locations as follows:
A. In the canopy of the large tree which grows on top of a building at Storm Watch (West of Dornogal).
*The branch is in the Southwest area of the canopy.
*When looking at the map, your icon will actually be on the wall closer to the door of the building.
B. Western lil peninsula on West side of Dar Oztan, underneath a small cluster of trees.
C. Under a tree at the West end of a small pond which lies almost smack in the middle of the area between the roads, Southwest of Fungal Folly delve.
D. Sitting on the base of a tree near a rock formation, along the edge of the cliffs South of Cinderbrew Meadery.
E. In the middle of the West-most island East of Dar Kazhad, crab is on the side of the rocks near the mountain peak, hiding somewhat behind a tree.
F. On the edge of the mountainside underneath a small grouping of trees, West side of the mountain where we meet Freysworn Letitia.
*When looking at the map, you will be somewaht on am iddle layer of the mountainside, directly North of the East-most leaves of the giant tree.

4. Return to Freysworn Letitia inside the cave and receive Hanna's Locket.

The cooldown on the Dalaran Sewer Turtle is loooong. Exactly now long, I'm not sure. I waited about 20 minutes and then ran a dungeon with my guild. When I came back, I was able to feed the next item to the turtle. I logged out and went back the next day around 10 hours later. No turtle. I thought had the last item in my bags but it wasn't there. I went back to my bank. Got both the Dornish Pike and Goldengill Trout that I had in my bank. I had fed the Fish and Chips on one of the interactions the day before. When I got back to Freywold Village, still no turtle under the tree.

One of many achievements that is making me pull my hair out as a 100% achievement hunter.
