The ogre that spawns is named Gor Grimgut. Non-elite 67, only special ability that I saw was his Furious Rage (+50% AP and 300% speed, I believe). If solo, make sure you're at full health and mana before you grab the spear.

Also, on your way there there's a quest giver at 44,72 who gives out "They Stole Me Hookah" https://www.wowhead.com/wotlk/quest=10542 which you can do as well.

The Thunderspike can be found at 39, 85. The entrance is at 42, 84. When you attempt to loot the spike, Gor Grimgut will spawn. Gor is a level 67 melee ogre with an enrage at 15-20% health. He drops the object you need to complete the quest. He also counts as a Bloodmaul Ogre if you have the following quest: https://www.wowhead.com/wotlk/quest=10505 (The Bloodmaul Ogres).

Pick up the quest (at 45, 72) before completing this -- both objectives are close to the cave at the southern end of the Bloodmaul Outpost.

Apparently the thunderspike object that you click on to summon Gor Grimgut despawns for 15-30 minutes after it's used so if you get to the end of the cave like I did and find nothing there to click, queue up for a dungeon or BG or something because you may be waiting a while, but it doesn't necessarily mean you're at the wrong place just because you can't find it.

Easy, in the back of the cave, looting it spawns a lvl 67 ogre, killing him drops it.

I stealthed in being a druid, watch the dogs though, they can sense stelth, just keep a fair decent distance away or run behind them when they turn (if you stealth).

Overall this wasen't too bad.

Getting to the cave from Thunderlord Stronghold:
Head south out of town and then go west across the southern span towards Sylvanaar. Skirt aroud the southen edge of Sylvanaar and take the path south a short distance then turn right(to the east) and head down the twisting cliff path.

Here you run into the your first Orcs. The path has at least one switchback that will allow you to jump down if you wish to avoid a few of the orcs. Once at the bottom hug the cliff edge heading south. There are typically no MoBs along here and you can go right to the mouth of the cave.

From the cave you will have to kill your way to the back or stealth in. Its pretty straight forward and you may be able to avoid at least 2 or 3 orcs even if you can not stealth. I think I ended up having to kill about 8 or 9 total from Thunderlord all the way to the Tunderspike.

Once you pick up the Thunderspike one Orc spawns. Up untill now they have all been level 65ish and this one is 67 I if I remember.

From there you can port out if you wish.

So where does Rexxar fit in? Does he now have a new quest for me? Is he in the outlands, too, or just in Desolace/Feralas still?

If the vision that I granted you is true, then you must travel down into the ravine and to the south end of Bloodmaul Ravine. There you will find the Draenethyst Mine.

Journey to the back of the mine, and retrieve the Thunderspike. Ignore everything else! I have no interest in the crystals of the mine, nor the ogres therein.

As soon as you have the spear return it to me here!

And, <name>, be very careful with it... I would hate to think what would happen to you should you break it.
