Coords / screenshot is wrong....

Go to the cave at 33.6 76.2 (according to cartographer)

If you already did the John Sterling quest where the bear kills and eats him you can wall hug past him.
Continue down into the bear cave. When the bear hasn't been eating him it's sleeping and you can walk right past it up the other slope and get the frozen heart. The screenshot of the heart is correct.

Actually, the coords are right but you have to go inside the cave first.

Map and coords are correct, just enter the cave as said above. When you first enter hug the wall on your left to avoid the pirate. The giant bear is sleeping , you should be able to ignore him too.

For people like me who don't quite get coords :P Just go down where the Reef Bulls are and go left considering your back is facing the sea. You'll see the cave, enter it. The humanoid you find inside will flee as soon as you down him to 8k health and the bear, Hozzer below will eat him. I went through the hole the elite humanoid went, but I jumped on the walls to avoid Hozzer instead of falling down. Then I just took the path up, sticking to the walls to avoid the huge bear. It's easier than it sounds ;)
