To complete this quest, kill Lord Llama Perpetua in southern Crystalsong Forest. You'll find him in the mountains separating Crystalsong from Dragonblight, to the east of the ruined bridge. He is a unique spawn, so you may have to wait. Kill him and loot the Ascua de llama perpetua. Fly to Winter's Breath Lake in northern Howling Fjord near the Taunka outpost there. The Doncella del Lago Aliento Invernal is encased on a small island in its center. Use the ember on her to complete the quest.

This quest, along with , and , is one of three weapon-fetching dailies available to Argent Tournament Aspirants and Valiants. Each faction has its own version, though they're identical aside from the specific quest NPC. This particular quest is for Silvermoon Valiants. The other versions are:

The rep reward from this (and other valiant quests) is actually 62 rep with all horde factions other than the one you take the quest with, 250 for the horde faction the quest is from and 250 for the Sunreavers.

For example: If you complete this quest as a valiant of Thunderbluff, you'll receive:

250 rep with Thunderbluff
250 rep with Sunreavers
and
62 rep with Undercity, Silvermoon, Darkspear Trolls, and Orgrimmar

Note that while I did not receive any in-game notification of the rest of the reputation gains, checking my rep bars before and afterwards it is clear that you do actually receive them. Completing all 4 dailies will award you 1000 rep with each of Sunreavers and Thunderbluff and 250 rep with all other horde factions.

I had a problem standing next to her and using the thing to break her ice. It said I had to be near the maiden (although I was right there.) I exited game, came back in and that solved it.

I had the same problem, until I realised i had not selected her. It may sound silly, but as the text does not say this, I didn't bother.

To sum up: If you can't use the item, ensure that you have the Maiden of Winter's Breath Lake selected.

In southern Crystalsong Forest, overlooking the Crystal Vice's exit, you will find Lord Everblaze. Kill him and take the everburning ember from his corpse.

Bring it to the center of Winter's Breath Lake in northern Howling Fjord, and use it to free the maiden encased in ice. She is the guardian of an ancient sword and will award it to whoever frees her.
