You'll need to use the vehicle on the bottom of that cave. Mobs start coming and the last pack has Dr. Terrible in it.

He can be located, by mounting up on Nergeld. He will appear when you start the fight.

You need to have the quest The Sum is Greater than the Parts.

When I tried to do this quest the mobs spawned then after a few seconds despawned, respawned, depsawned for about 4 times, then the "boss" came, despawned and never came back..
