This is one of a handful of items in-game that actually lowers a stat. This particular one is fine for melee classes but not for casters.

I'm wondering the same thing. why does this thing have minus 3 spirit? why would I want to use this? Is there an advantage in having -3 spirit ?

The item is called Tear of Grief, greif would make you sad. Hence -3 spirit. As for the intellect buff I guess it might be suggesting that when someone dies you are made aware of your own mortality, and being aware of your mortality you would take more precautions so as to not die. Or might not.

u dont want it -3spirit for +5 Intellect is kinda a bad deal getting more mana while harder to regain will waste your leveling time

A number of comments are querying 'why a penalty to Spirit'?

It's a trade-off (will the benefit on one stat outweigh the penalty on the other?); and many games leverage this concept as something which adds to the decision making process.
For the most part WoW has traditionally avoided this kind of thing; and left decision making as purely good < better < best.
This item is one of the few exceptions where the game does present a "tough option with consequences". (ftr, the current retail expansion BfA has significantly changed their usual approach with its 'corruption system'.)

As for this particular item, any class that rates Int as much more important than Spirit could make use of it.
However, 'buyer beware': Spirit is usually much more valuable than most people appreciate for reducing down-time between fights.
E.g. A mage clearly benefits from the Int and gains more mana as a bonus; but may ultimately end up spending more time drinking to recover mana.

But for some classes it really does have potential:

A comment above explains the thinking for a warlock.
Also, as a druid at this level you'd probably spend most of your time in bear form slowly regenerating mana. A big mana pool makes it easier to switch to caster form, cast some spells, and have enough mana to go back into bear form. So bigger mana pool is probably more valuable than speedy mana recovery. The item definitely has potential for a druid. (Though I suspect you'd see better mileage with a good 2H stat stick.)

Of course the other option is to make use of gear swapping: +5 Int is nice for any spell caster during a fight.
During down-time, swap/remove this item and get faster recovery with a temporarily higher spirit.

This may be the only item i've ever encountered to have a negative stat in all my years of playing since 2008.
