On the plus side seems like there wont be a token for each piece of armor. One piece will be able to buy one of the 5 pve pieces.

If this one token is able to buy the Helm, Hands, Chest, Legs, and Shoulders I think blizzard should change the icon and name to something such as a Token. That way people don't just assume its for a chest piece and such. I was at first confused about this also. Of course people will be using this token probably in this order:

Chest>Helm>Legs>Shoulders>Hands

to recieve the most benifit while buying the most expensive pieces first.

These do not drop from bosses in 25 man heroic. You need to complete tribute runs to get them, which means a maximum of 25 unsuccessful attempts (all bosses added) before killing Anub'arak.

Does this item drops only in 25 man tribute run, or also in 10 man ?

This item drops only from the chest at the end of Grand Trial 25. You get 2 random tokens no matter what and an additional 2 bonus ones if you finish with 45 or more tries left.

first of all, as far as i know, There is (there's also an achivement for each) "Call of the Grand Crusade" which drops 1 of the 3 armor tokens, that's just for completing heroic mode, 25 man. (10 man would be 2 Trophies of the crusade.)

Then there's "A tribute to Skill" which is with atleast 25 attempts left, it drops (i think) all of the above, PLUS the 25 attempt loot, The spine breaker and other equipment, swords, etc. (10 man and 25 man loot is close to the same loot, just better stats).

Then comes "A tribute to mad skill" which is for 45 or less which, for 10 Man, would be 2 Trophies of the Crusade and all of the above (10 man)
25 Man would be All of the 25 man drops above + an extra armor token (2 in total)

Then comes "A tribute to Insanity" which is all 50 attempts remaining, which, as alliance for 10 man drops capes and also a swift steed mount.(also all above for 10 man)
25 man is, again, all above, but, weirdly no mount...also the capes as above, but better stats.
All items dropped are Heroic items.
