I picked this quest up on Heroic, and for some reason none of the mobs in the instance dropped any bandanas. However after leaving and coming back on Normal all the mobs started dropping the quest items.

My guess is that you have to be on the quest when the mobs are generated in order for them to drop the bandanas.

This quest was added in patch 7.1, and unlocks the Defias Leather set. To obtain it:

You're going to need a Rogue in Outlaw specialization. You don't need to be level 110.
Enter The Deadmines Normal
Reach Ironclad Cove, the area with the wooden catwalk and the big boat (near the last two bosses)
Optional: if you don't have a Green Wing Macaw keep killing Defias Pirate until you get a Parrot Cage (Green Wing Macaw) with one inside
Locate the Ghostly Parrot, found at these coordinates
Summon your Green Wing Macaw and stand close to the Ghostly Parrot, an extra button with a Grappling Hook will appear. If you click it you'll climb the wall
Accept the quest Deadmines: Sea'in Red from Captain Bramblebeard. He's gonna ask you to obtain 100 Blodied Defias Bandanas from the enemies inside the instance. If you already killed them all just reset the instance to complete it (remember that you need to do it in normal difficulty)
Turn in the quest and enjoy your Defias Leather set!

I was able to get this quest on my level 65 rogue. I assume you can get it at an even lower level, just as long as you have the pet and are in the outlaw specialization.

For some reason I learned the transmog before completing the quest and was unable to use the ensemble. It works out either way, but not sure why it happened.

Are ye up for a plot of revenge and skullduggery? These meddlin' Defias thugs murdered me and me crew years ago before they took over this cave. I propose a series of counter-murders to bring about balance!

Bring me proof ye've dealt with enough of the thieves to clog a bilge pump!

Okay, there's a short guide:
Some thing you need to know before:

First, Ensemble: Blackened Defias Armor can be obtained by only Rogue, Outlaw spec
You dont have to be 110 level to be able to pick up the quest Deadmines: Sea'in Red
There are five items in set: Blackened Defias Armor, Blackened Defias Leggings, Blackened Defias Gloves, Blackened Defias Belt and Blackened Defias Boots
You dont have to be rogue only to transmog the set, other leather wearers are ok too

Fine, now the guide:

Go to The Deadmines instance, entrance located in Westfall at 25.4 51.0 point (there's long way through the underground

Run normal and proceed till you found a giant cave with ship inside it (Ironclad Cove). Also, skip all mobs, slay only bosses to open doors
When you come into Ironclad Cove, turn left, there you'll see a ghostly parrot far away, above a corpse next to a water
Come close and summon battle pet — Green Wing Macaw. You can obtain it at AH or killing pirates on the ship — Parrot Cage (Green Wing Macaw). When you summon the pet, there an extra button will appear — Grappling Hook. Use it to reach a hidden place, where Captain Bramblebeard is
Pick up Deadmines: Sea'in Red quest. Bloodied Defias Bandanas have 100% drop chance from each mob (including monkeys and parrots, lol). There are enough mobs in instance to get all 100 within a single run reset (if you skipped them, ofc)
Get back to Captain Bramblebeard and turn in the quest, use Ensemble: Blackened Defias Armor to get full set. Visit transmogrificator to see what you've got

Just an observation, but the ensemble vendors for 750g.

Two things happen at level 10 for a rogue: first they can zone into the Deadmines. Second, they can pick outlaw as a specialization.

Now I'm not suggesting this is a very time efficient way to make money, but its basically a free pile of 750g for every rogue you level to 10, as long as you have someone to run you through VC.

I just wanted to add an observation here. I ran this on Normal and was able to loot bandanas off of everything including the chained up monkeys.

I was able to collect all 100 bandanas in a single run just by killing everything I came across including all of the parrots.

I haven't seen this mentioned anywhere yet. If you return to the ghostly parrot to head back up and turn in but the extra action button doesn't appear, just dismiss your Green Wing Macaw and re summon it. I hit that issue and that caused the button to show up so I could finish.

So I got the quest but for some reason I can't get any bandanas. I've run through it twice with absolutely nothing. Any ideas as to why this is happening?

Not 100%, but I'm pretty sure you can't get this unless you have Legion. A friend of mine and I are leveling through RAF and found out about this so decided to run it. He doesn't have Legion yet and wasn't able to see the quest. I don't know if they're related or just a bug that he can't see it but going to try again after he get Legion.

Any reason why I couldn't pick up this quest on my human? Had to do it with my undead

With level scaling impletemented on the last patch, this dungeon scales all the way up to 60, if you are low level and want this outfit just queue normally on deadmines, do the steps that others have pointed out, now, a full run with a random group yields like 10-15 bananas becouse not all mobs are lootable while in group, instead go manually to instance and use the cannon in the first room with mobs, you can kill the ones right on the side of cannon, the ones at start of right corridor and the ones near room entrance. I was able to get 10 bandanas each time, when cannon is destroyed just walk out of instance reset and do it again until you have 90 or so, then just queue to finish gathering the reamaining bandanas and turn in quest.

I picked up this quest as an 120 rogue on Normal mode, then completed it on Heroic. Did have to resummon the macaw before the grapple button reappeared on Heroic, but it IS possible to turn it in on Heroic.

The Ensemble: Blackened Defias Armor is obtained via completion of Deadmines: Sea'in Red. Only rogues can do the following questline, but the Defias armor set can be worn on any leather wearing toon!

Step One: To receive this ensemble, you must first be in Outlaw specialization and have the Green Wing Macaw pet.

Step Two: After you have acquired your companion and changed your specialization to Outlaw, head over to The Deadmines in Westfall. Make sure that you have set the dungeon to Normal before entering! Mobs will not drop Bloodied Defias Bandana on Heroic difficulty.

Step Three: Once inside, make your way to the end of the dungeon. It is recommended that you do not kill any of the mobs aside from the bosses, the reason being that you will be killing and looting the mobs for the quest you receive later on at the end of the dungeon. Upon reaching the end of the dungeon, run along the boardwalk until you get to the area called Ironclad Cove where you will see a Ghostly Parrot.

Step Four: Walk up to the parrot and summon your Green Wing Macaw, which will then grant you access to the Grappling Hook extra action bar. Using this extra action button will take you up to Captain Bramblebeard, who will then grant you the quest to slay Defias in the instance until you have gathered 100 Bloodied Defias Bandanas.

Step Five: Slay all the Defias! If you killed Defias on your way to the Captain prior to starting the quest, simply run out and reset the instance. Resetting the instance will spawn all of the previously killed mobs, allowing you to kill and loot them again.

Step Six: After you have obtained 100 bandanas, head back to the Ghostly Parrot and summon your Green Wing Macaw to once again be taken up to Captain Bramblebeard. Turn in your quest and you will be granted the Ensemble: Blackened Defias Armor.
