The Many-Eyed Goat seems to reference the "goat with seven eyes that watches from outside" that the Puzzle Box of Yogg Saron mentions occasionally. This could also be linked to Baa'l, the demonic goat companion pet being added. All of this is purely speculative, however. We'll see how this plays out when it makes it to live.

it's a reference to the Lovecraft deity Shub-Nigurath, symbolised as the a black goat in a forest with a thousand young. Y'shaarj matches the desceiption.

You can turn in this quest in the lvl110+ phased zone of Silithus (the one with Sargeras' sword). You don't have to talk to Zidormi to go back in time, and there should be a portal to Silithus in Dazar'alor for Horde and Boralus for Alliance. You need to turn in this quest to get credit for Tell Me A Tale.

I always assumed the 7-eyed goat referred to Y'shaarj, due to him having 7 heads. But this vision, received from the mass of a random faceless, apparently implies that who or whatever has 7 eyes is still active, or at the very least has echoes that are still affecting the world.

Since nobody's talking about how to get this quest item, it's a rare drop chance from any island with Old God enemies present. You are sometimes guaranteed to find Old God minions on one of the three islands available in a given week as they can be the primary invaders of two of the possible islands. If you want to farm this quest item, weeks that they are the primary invader in one of them are the best for doing so.

I did Crestfall with Pirates and had a faceless invasion. Killed all faceless, one in special was Herald Razzaqi, because i haven't see it before. Can't be 100% sure of it because i killed other 2 faceless rares i didn't remember the name, i killed those rares before but Razzaqi and nothing dropped.

This was the last quest item i needed for the meta achievement, decided to get it out of the way before Shadowlands release. It seems old gods used to be a standard spawn some weeks at the start of BFA, however in 2020 they have never been standard present on any island. The only way is to buckle down and farm islands for a random faceless rare or a faceless invasion.

It took me a total of 386 islands over the course of three weeks, of which 13 had a faceless invasion. All of these were done on heroic. The longest i went without seeing a faceless invasion between islands was 61, the shortest was 3. I dont think it matters but just to be sure if i had a faceless invasion i made the effort to kill as much of the rares as possible, i killed 3 on the actual island where i got the drop. No portal to a faceless boss was present.

I only saw 21 faceless rares as random mob spawns on an island without being part of the invasion.

Had to stop my Shadowlands experience and grind islands for 10 hours straight.
After doing around 15 Havenswood runs on Heroic, I finally managed to form a group of 3 and signed Mythics.
2nd game was Havenswood (mythic) and, yay, it finally dropped!
Now only Druidic Fur from Druids of The Claw remaining...

Important note: If you manage to form your group and farm islands. I suggest you do the following, to boost your chances:
-sign heroics until you encounter Havenswood, then switch to Mythics until Havenswood is up and vice versa. Like this you might get Havenswood 6/10times if you are lucky.

After killing a lot of old god rares, I finally killed Herald Razzaqi and got the item. It is highly possible that this item only drops from him.

Today, i found this item after killing two rares and a couple of normal minions during a Faceless Invasion in a Crestfall with Orcs as the primary enemies.
I don't remember the name of the first rare, but the second one was found inside a portal. It was General Shuul'aqar.

Sorry for asking that trash question, but i look @Wowhead frontpage nearly every week and i have never seen Old Gods as front enemys on any island.
So my Question: Can Old Gods be the front / main enemys on an island?

ATTENTION: Yes faceless or old god minions as principal in Havenwoods exist! it is up this week, probably takes like 1 year to be up, so, yeah I farmed like crazy for Wriggling Mass because faceless won't be up as principal for a long time again. And guess what? it dropped without rare Herald Razzaqi up but a rare something like harbinger was up, so, I think this item is not related to any rare, it is just random, another info is a mate and I was farming the same item and I got it and he didn't, so, it is really RNG!

Got this on Mythic solo queue, when faceless were the main enemy. Notably I only killed one Faceless rare (on top of the trash), and it was NOT Herald Razzaqi (it was a long/wierd name I can't recall). It probably drops from any rare faceless kill.

if you are soloing, there is an easy way to farm these quest items. First, make 2 other free trial accounts under your battle.net. (Make sure they are under the same battle.net as your main one, default name are wow1 and wow2). They are free to play, and can be leveled up to level 20. Make 1 toon on each of these 2 free accounts. They are eligible to loot most of the items in game like mounts, pet and toys.
Now, group up with your main toon and run the normal island expedition. Just using your main to kill mobs and usually take 5-10mins to finish each one. At the end of the run, all of these 3 toons will be eligible for rewards and now you just increased your chance on mounts, pets, toys and the quest items by 3 times since all these 3 toons are under the same battle.net.( all mounts pet and toys are account bound). Lower your graphic settings or use different electronic device to reduce lag if needed.
heroic and mystic island required all party members to be level 50+ and higher. So this method is only used for normal island expedition. the only different I have noticed is the amount of coin is lower than running mystic version. Other rewards are still dropping like running mystic.

Finally got this q item after ages (years!) of farming. Tried all sorts of methods, farming Havenswood on a week when it was up etc.
This week Havenswood was up but during one of the other islands, Crestfall, I finally had a faceless invasion, Herald Razzaqi was present.
At the end of the island the wriggling mass dropped, was very happy that the grind was over. And indeed, couldn't hand the quest in without party sync with a lower level.
It's a %^&* grind but you will get it in the end!

Received this after an expedition with faceless as the primary invaders.

The only faceless rares that I saw on the island were Harbinger Vor'zzyx and My'lyth, for anyone who thinks that particular rares "drop" this item.

After running what feels like thousands of islands to get the last couple of quest items for Tell Me a Tale, this finally dropped after I killed a named mob - Vudax. I hadn't seen this name mentioned before so thought I would add it in. Good luck island adventurers!

Description

<The tendril writhes and pulsates in your hand as your mind is assailed by all manner of vicious thoughts. One vision in particular surfaces in your mind again and again. In your mind's eye, you picture a many-eyed goat laying in a pool of black water.
