I am privileged to be the very first to make this comment on this daily especially after I died just once. I tried to figure this out and quickly found the obvious way, so here it is..

After you have entered the area and engage Itoka he will stomp out tonnes of aoe boulders in your way, strafe away from them and watch out for the lightening charges within the red circles that wander slowly around, they will do a lot of damage but are easy to avoid.

P2 - He will begin to cast out his protection shell so you will have to take out all four conductors that keep him there again avoiding the circles that move around. So if you walk on the rocks and stand on the ones that look like grave stones you will be able to go right up to each conductor and switch them off via the cog until all four are done and Itoka is released from his shell. (WARNING, do NOT tread on static electric or you will probably die within 5 seconds !),
Itoka will then commence his fight with you but you will still have to avoid his aoe damage abilities till you have nuked him down.

Note;- He only casts this electric once, apart from that he doesn't do any real damage ( as much as the rest of the quest bosses that are round there ), as long as you follow these steps you will be fine and should be a fairly easy and straight forward fight.. Have fun!

Short summary - take your time during the bubble phase. There's literally no rush, and stepping in the water instead of on the stones will totally kill you, and thus slow you down.

For some reason, after completing the Maximum Capacitor quest, I did not recieve this quest as a followup.

If you manage to abandon/not accept this quest, it is actually given by Scout Captain Daelin at his location in Conqueror's Terrace after you have completed the previous part in the chain.

When completing this quest, I STRONGLY recomend killing Fleshcrafter Hoku first to obtain the Mogu Codex Fragment. Killing Fleshcrafter Hoku gives you the opportunity to gain Blood Infusion by killing Possessed Blood. This buff gives you +10% damage and stacks up to 12 times. This makes the first phase of Itoka fly past, especially if there is a group of you killing him. Best way to max your stacks is to stand on Fleshcrafter Hoku when he starts summoning the Possessed Blood and just AoE the living %&$* out of them. Once he is dead, run straight back to Itoka and let rip. As bookmarker states below, the bubble phase is a time to sit back and re-gather yourself, but unless you have people ready to deactivate the pylons, you will lose the Blood Infusion by the time the third phase starts.

I had already killed this NPC prior to picking up the quest to gain access to the forge. Now for some reason, the is not dropping and the quest "Forge Ahead!" is hung up. Any ideas?

Seems to be a bug in 8.1 that doesn't let you actually auto-accept this quest. You can however still pick it up from whatever hub you picked up "On Her Magic-ey Secret Service" from or whatever quest led you into the mines.
