The skulls are found inside the main base. They give random amounts, most I got from a pile was 22. So it's pretty easy.

Its in the building on the right when inside in the middle should be a steep circle with markings on the ground. Click on the skulls then you're avatar would start throwing them in the circle. When you are done in the middle should be a small little tornado (mages and hunters you may want to stand back). Then Nazargan will appear in the middle when you kill him the quest should be complete.

Nazargen may be summoned in the center of the building found at 55.7, 28.4.

Once you get all the skulls, the summoning circle is in the center of the main keep. It's not in the same building you got the ritual letter from.

I received 54 skulls from the x4 piles in the first room you get this quest, and 25 from the pile near the building entrance on the way out. There are also other piles on the way to the main building's entrance scattered around the courtyard. Another four piles are to the right, just inside the fortress gate and side of the main building.

Nazargen is only a 31, with a few more HP than the normal 31 but no special tricks. Don't loot the mobs you kill until you're on your way out and there won't be many annoying respawns.

If you're here just to get those nice blue rewards (as I was) and have not quested in the area, make sure to do the quest chain starting with My Time Has Passed to unlock this quest chain.

If you have extra skulls at the end of this quest, do not discard them before turning the quest in. The quest will be abandoned and you'll have to trek all the way back to Nijel's Point to pick it up, then back to Thunder Axe to collect all the skulls and kill Nazargen all over again.

After 5 tries, i'm now sure, this quest is currently (patch 6.0.3) bugged. Nothing happens when you place the skulls.

This is currently bugged, using the skulls does not summon the demon. After failing a few times I tried to dismiss my pet before summoning and that worked. Worth a shot if you are a pet class.

This quest may take you a few tries. What others have said is true. You must be in the centre of the circle for this to even initiate. If you move at all after using the skulls, the demon will not be summoned.

Finally, even if you do everything correctly, there is a chance it still will not appear. Keep trying though, as it will work eventually. You will know when it has worked, because the skulls will generate purple fumes that tag your char. Stay still, and wait a moment longer, and the demon will arrive. Again, if you move you may interrupt the summon.

Quest is not bugged, completed just now. You should be "right in the middle" of the inner circle.

Quest is able to be completed. After you've collected 100 skulls wait until the Shadowmage is in the center of the ring. Do not attack him and then initiate the summons standing pretty much on top of him. Wait a few seconds for Nazargen and then this will not bug.

To complete this quest, do the following:

Collect 100 Bleached Skulls from the Bleached Skullpiles.
Go to the big building at 55, 26 (map).
Go to the Summoning Circle in the middle of the building and click on your Bleached Skulls (Summoning Ritual).
Nazargen will be summoned. Kill him.

Anyone still having a issue with this bug? Once I use the bleach bones and stand in the middle of the circle, Nazargen still does not appear after 6 tries.

I found this quest to be really disappointing. Whenever my character gets a bright idea to give herself a quest, I always expect it to backfire terribly. That goes double for a quest like this where my character goes "oh, they're summoning a demon? The best way to solve this problem would be to summon the demon myself!"

I was expecting the demon I decided to summon to be way too strong for me, and I'd go report back to the NPCs, and they'd say "You did WHAT?" and then I'd have to clean up the mess I just made. Like it turned out they were waiting for reinforcements before going to stop the summoning themselves or something.

This zone is just full of my character having the worst ideas ever and then having it all work out well for me in the end.

Collect 100 Bleached Skulls from Thunder Axe Fortress, place them at the Burning Blade Summoning Circle, then kill Nazargen.

Description

So it's true....

The parchment you discovered in Thunder Axe Fortress details a powerful summoning ritual. One stronger than the Burning Blade have ever been known to possess.

Clearly, this rite must be stopped at all costs!

Perhaps if this demon were summoned forth prematurely the crisis could be averted.
