For Alliance (in the PTR as of 13 January 2015) sold by Krom Stoutarm in the Library of Hall of Explorers in Ironforge.

Cost is 1140 gold at Friendly rep and the NPC is affiliated with Ironforge which indicates it will probably be discounted with rep.

This token is used to upgrade all heirloom weapons to scale to lvl 35. It can only be used once as it is consumed when applied to an heirloom in your Collections tab.

It is permanently sold by Estelle Gendry (H) and Krom Stoutarm (A) for 750g, by Galra (H) and Liliana Emberfrost (A) for 14 Mark of Honor and by Dame Evniki Kapsalis at the Argent Tournament for 65 Champion's Seal. It is also one of the rewards from a quest chain, which is given at lvl 100 by Drix Bassbolter (H) and Fix "Smallie" Biggswrench (A) at your tier 3 Garrison.

Finally, it can also be obtained during several world events:
- Darkmoon Faire: Sold by Daenrand Dawncrest for 120 Darkmoon Prize Tickets.
- Love is in the Air: Sold by Lovely Merchants in every capital city for 200 Love Tokens.
- Brewfest: Sold by Blix Fixwidget/Ray'ma (H) and Belbi Quikswitch/Larkin Thunderbrew (A) for 300 Brewfest Prize Tokens.
- Timewalking events: Sold by Cupri (TBC), Auzin (WotLK), Kiatke (Cata), Mistweaver Xia (MoP) and Tempra/Kronnus (WoD) - depending on event - for 900 Timewarped Badges.

This is how the upgrade process works - it's applied to the icon in the Heirlooms tab of your collections window (where you also find mounts, pets, and toys), not to the heirloom items themselves. That means that you only need to buy one per named heirloom and then you can create multiple 'instances' of the upgraded heirloom without having to re-buy the upgrade item. Any existing instances of that heirloom will be automatically upgraded also.

Item upgrade levels and user levels can be somewhat confusing. Also, weapons require a different consumable to upgrade them, while armor has its own consumable for upgrades. There are also two different of upgrade consumables for weapons and armor to get them to levels one and two which can also be confusing.

"Casings" upgrade armor.
"Scabbards" upgrade weapons.
Don't confuse the "Timeworn" from the "Ancient" casings and scabbards.
"Ancient" upgrades an heirloom to the first level and is usable for characters level 1-90.
"Timeworn" upgrades an heirloom to the second level and is usable from 90-100.

I got my ancient scabbards and casings at Darkmoon Faire using my Darkmoon Prize Tickets.

Vance, a user who posted on the Bliz/Battle site says that "Ancient Heirloom Armor Casing and Ancient Heirloom Scabbard sold by Krom Stoutarm in the Library of Hall of Explorers in Ironforge and by Estelle Gendry in The Rogue's Quarter in The Undercity. . . . From what I can tell, they have no level requirement. They simply cost gold. . . These are not to be mistaken for Timeworn Heirloom Armor Casing and Timeworn Heirloom Scabbard , which power up your heirlooms up to level 100. These can be purchased from the aforementioned vendors or obtained by completing a Garrison ."

He also says that a freshly purchased heirloom has 0 of 2 upgrade levels. Using an ANCIENT "will get it to 1 of 2 upgrade levels, which makes it usable up to level 90." Using one of the TIMEWORN items "requires a 1 of 2 upgraded heirloom and ups it to 2 of 2 upgrade levels, which'll last until level 100."

You want to know, how much gold needed for an alt?

First of all, you will need minimum 3900 gold to get the starter heirloom set which will gives you a boost till reach level 60 (+you will need a guild too, to get some of them which are listed below):

So, If you want to start, your alt character with +xp boost, you will need minimum: 3900 gold - Without weapon

After reaching level 60, you will need additional 1000 gold for Ancient Heirloom Armor Casing. This item is needed to upgrade your armor to 90, but please note: all of the items, waht you want to upgrade need one of this item, so to upgrade your full sett to 90 from 60, you will need: 7000 gold for the armor upgrade!
For the weapon upgrade to 90 from 60, you will need an additional 1200 gold for Ancient Heirloom Scabbard. This item is use only by: Two-Hand, Ranged Weapons and Staffs.

You want to continue your jurney with heirlooms? You will need a new type of the upgrade items!
This upgrade items works like the 60-90 ones, but this will boost your gear up to 100!

I have six of these in storage from the Misprinted Draenic Coin quests. I have most of the weapons I am missing Spear of Rethu which starts able to go to level 100 and Brawler's Razor Claws which I could use this on. I hate to trash them because when I do Blizzard will do something that would have allowed me to use them.
