This is a calling quest. Calling quests encourages players to engage in various content in different areas in Shadowlands. At any time there are 3 callings available, and all players, regardless of Covenant has the same objectives, eventhough the name of the quest can vary.

This specific quest ID is for players who have chosen the Venthyr covenant. The quest is both started and ended at The Accuser at the lower floor (through the mirror) of the covenant sanctum, Sinfall. Other players will have the same objectives with the following IDs

Kyrian players, see Challenges in Bastion.
Necrolords players, see Challenges in Bastion.
Night Fae players, see Challenges in Bastion.

To complete this quest, either travel to Bastion and complete the specific world quest Disloyal Denizens which can be found in the north-west corner of Bastion, or complete any of the dungeons that are found in Bastion, which are The Necrotic Wake or Spires of Ascension, on any non-keystone difficulty (Normal, Heroic, or Mythic).

I am Ventyr and while waiting on FP to Bastion my heroic dungeon queue popped for https://www.wowhead.com/spires-of-ascension

After I left the dungeon, I looked at my quest log and I saw it completed both https://www.wowhead.com/search?q=a+call+to+bastion and Challenges in Bastion

In the Shadow of our Failures currently causes some phasing issues for anything provided by The Accuser. i've been lazy about doing the campaign questline and now i can't pick up this calling quest because the NPC is phased to another location! you also can't pick it up from the version of The Accuser who provides the questline at darkfall tower (The Accuser), meaning you may have to complete the entire chunk of quests to make the NPC reappear in sinfall.

eta: the calling quest can be picked up after turning in Blackbale Betrayers, Maldraxxian Weapons, and There's Always a Paper Trail, at which point The Accuser returns to sinfall.

To complete Challenges in Bastion you need defeat Valinor located in Bastion when the Weekly World Quest Valinor, the Light of Eons is up (weekly rotation with another World Bosses).
To defeat this guy you'll need a group.
I got Memory of Archbishop Benedictus on my spriest (first try).

The "Challenges in X" Calling quests can be completed in Mythic + Dungeons (in addition to lower difficulty dungeons but I could not find it listed anywhere if Mythic+ dungeons awarded credit so I came here to fix that) relevant to their respective zones.

For Kyrian: If Xandria is not there to turn in this challenge, abandoning A Perilous Journey will make her reappear at that location. After you turn in the Challenge, you can retake A Perilous Journey.

Challenges in Bastion

Complete the Disloyal Denizens World Quest, the Spires of Ascension Dungeon, or the Necrotic Wake Dungeon in Bastion.

Description

Our allies in Bastion have long served the cycle of the Shadowlands, delivering souls in need of our services. Yet, within their ranks, doubt has grown and festered. If this continues, we will all pay. Their loss of focus shall not be allowed to continue. We will punish their wandering hearts. I accuse these souls of sins against the Shadowlands. Mete out justice in the name of Revendreth.
