Twitching Eyeball is available if you finish the Xal'atath questlines in 8.1.5 and opt to cleanse N'Zoth's gift.

If you do not cleanse the gift, you do not get this toy.

but you have a permanent eyeball on your head, and can see everyone else that secretly kept the gift

Only way so far to keep the nzoth buff is to never turn the quest in. I'd like to keep the nzoth buff but I went ahead and turned in the quest. I received a toy and the buff was removed.I guess my alt can be the nzoth follower.

If anyone wants to remove this "gift" from my head, they can do it over my cold dead body!

I have decided to keep this "gift", it makes me feel special in the eyes of at least someone.

Horde side has a choice, I was expecting the same from Alliance side too, but only way to stay a N'Zoth follower is not to give the final quest, if you accept it just gets removed. I got the toy with my main already so I'll be keeping this one to see how mad my Human Warlock will go :)

Hi all

To Start this Story line You need find Azsharan Medallion, this is droping from mobs on World Quest Naga Attack! ( on all of them ). After you will get Azsharan Medallion you will start storyline.

2. Orders from Azshara
3. Every Little Death Helps
4. Unintended Consequences
5. The Pirate's Treasure

8. His Eye Upon You Reward from this Quest is Twitching Eyeball.
9. Crucible of Storms: Relics of Shadow To finish first they need to open Raid ( stil locked :( ).

Okay, I'm confused slightly. On hordeside, clicking on the "Actually, I'd like to keep this" option just closes the window and doesn't progress the quest :/ I don't know what to do.

I want to keep the curse Hordeside because I heard that you don't even have the choice Alliance side, so my Alliance main will have to go with the cleansing.

I had a hard time finding out where to turn in this quest as Horde. If you are Horde, to locate the Altar of Akunda, go up the stairs to Zanchul. Once you are on the platform where the Bloodraged Pterrordax are, go right. Pass the Altar of Bwonsamdi on the left and go down the stairs. When your map changes to Terrace of the Chosen at the bottom of the stairs, Altar of Akunda is on the left.

Listening to people talk, I thought I would be able to choose whether to side with N'zoth or have the curse removed, in much the way horde had a choice to betray Sylvanas and help Saurfang. It turns out, the way you "side with N'zoth" is to simply not turn in the quest. Not turning in the quest will bug me until the end of time. Will this eventually need to be turned in anyway? Will there be consequences later in the story if you do turn it in?

You CAN redo this quest chain on an alt and pick the other option if you want to see what happens to your character depending on which side you choose. Just keep in mind that if you want to keep this buff, do NOT choose the option to remove this curse. Simply remove the quest from your quest log. There is no way to currently remove the quest from showing up on your screen if you choose the option to keep the buff.

To obtain this quest you need to find an Active World Quest: H/A: Naga Attack

Note: obtainable at level 120*
Note: Final quest gives Quest related to complete and kill boss inside the raid. (Reward?)

How do i obtain the QUEST?

Find World Quest Naga Attack in Kul Tiras or Zandalar
Keep killing NAGAS Until the quest item drop (70% drop chance).
Once done go to your main Tortollas Headquarter and countine questline.

** Horde **

Note: Will be using (Alliance-Links) as Guide Line (below).

Quest Part ONE: The Azsharan Medallion.

Quest Part TWO: Orders from Azshara.

Find the altar.
place the Xal'atath, Blade of the Black Empire on Altar.

Quest Part THREE: Every Little Death Helps.

Go and slay around 10-14 Nagas and your done.

Quest Part FOUR: Unintended Consequences.

ALLIANCE:.. Take Flight Path to Anyport.
HORDE:..... Take Flight Path to Anyport.
Mount and run up to the road, Head left (north).
Soon enough you see abit of a Ruin wall, head in there.
Find the Void stone, Kill Xal'atath, Loot Orb, Turn in Quest.

Note: If you cannot do anything, Just wait a moment since someone just did the event before you (takes max 3min to reset).

Quest Part FIVE: The Tempest Crown.

Go to Vol'dun.
Mount up and run near the Coast, all the way up to the Hyenas.
Once you are entering the very first Hyena pack, Head toward left you see a Mountain-cliff block.
Jump down into the water/beach, you will see a small kinda Cave Entrance.
Head inside, Kill the big CRAB, Turn in Quest.

Quest Part SIX: Twist the Knife.

Take Flight path Mildenhall Meadery to Stormsong Valley at the cordinates "83.8, 47".
Find the NPC: "Xal'atath outside the raid entrance, Speak to her.
Enter the raid (Solo-Scenario).
She will ask you to Place 3 items, Void Stone and Trident of Deep Ocean aswell Tempest Caller.
After that she betrays you, also initiates a Tiny Video-event.
You recived the Gift.
Return to Boralus, Find Brother Pike at Harbourmaster Office, hes behind Quartermaster NPC-lady.

Quest Part SEVEN: His Eye Upon You.

Leave Boralus, Head to Stormsong Valley.
Take Flight path: Mildenhall Meadery, Get over toward the bottom side of the east Isle, Follow road up.
Speak to Brother Pike, He will start Infuse your body and remove the Curse.

NOTE: Reward: Toy, Twitching Eyeball

FINAL Part EIGHT: Crucible of Storms: Relics of Shadow.

Find a Raid Group NM/HC/Mythic - or LFR, just anything you like (Scroll down for release date).
Kill the final boss and you have completed the quest, Congratulations.

You can choose to have it removed, but don't turn in the quest as a way to toggle the curse. If you want it back just abandon the quest. Was missing my transmog and decided to give it a shot to see if it worked and I'm glad to say it does.

It may be evident to others but when I got the quest I did not know what to kill. So I was killing the mobs in the area. At the time it was not naga because I noticed that there were others killing naga (did not dawn on me that it was for the same quest). Anyway, I looked here and it said kill nagas so I went to where I knew there were nagas and it did nothing. I came back here and found that it had to be nagas in this area. At that point there were nagas around and I killed them and I think after 10 I was done. So if no nagas to be seen just wait for respawn.

I made a ticket to see what a GM would say about us needing the buff later on. Here is the response…

This quest line is definitely designed to be mysterious so you’re not missing anything here. If you’re apprehensive about removing the buff, the developers have done their job because that is exactly what they were going for.

Currently the buff gives you an eye on your character’s head, and it lets you see other players and NPC’s who also have the buff.

We can’t help you make this choice. If you want to remove the buff, you just have to complete the quest-line. If you don’t want to remove the buff, abandon the last quest.

If you’re curious about both out-comes, I suggest leveling up an alternate character. That way each character can choose a different path!

I really appreciate your patience on this and I hope my response was both helpful and thorough.

So far, known NPC’s who have the buff are (credit given to Minsiem);

-Desperate Citizen inside the Great Seal in Zuldazar:

“Behind my eyes... I can feel their teeth. They nibble, they bite, they tickle my brain. Get closer, sweet one, let them taste you."

-Casteless Zandalari near the ship in Port of Zandalar:

"Mommy tried to send me off to meet his voice, so I could play forever.
Daddy had another plan, now mommy will never age again."

Additional Info (credit given to lxion87): You can remove this buff/aura by completing the quest: His Eye Upon You or His Eye Upon You. If you want to keep this buff, do NOT choose the option to remove this curse. Simply remove the quest from your quest log.

If you abandon the quest - does the eye remain on your head?

I kinda wanna keep the eye (especially if you have Dark Edge of Insanity as tmog) , but at the same time don't wanna have quest sit in log and bug me.

Can anyone 100% confirm please?

If you dont like the "eye hat" but love the buff and voices, there is a way to keep the voices and make the hat invisible. Use vrykul drinking horn, remove its buff and your normal head is visible without the eye. In nazjatar this is permanent unless you transform your character. In other regions it stays for about ten minutes like this before you have to use this trick again.

After keeping the eye of N'Zoth the previous day.

was completed by me. However, the game was reversed and the protagonist is a crab. The new objective was to eat 20 baby turtles. That was a fun change... and rather satisfying to see Blizzard go back to its roots of some quests being flat out evil.

For instance, the vanilla quest for ; That was to gain the as a pet for Horde aligned requiring two from huntind down 2 NPCs in who were 'good' people by definition.

Just in case anyone else wasn't sure of it: you WILL get the toy if you complete the quest now, in 8.3 (and likely later).

A few comments on this quest chain sounded like it'd only work if one completes them before the next major patch. Not sure if it's still possible to begin the quest chain, but I had in my quest log ever since 8.1 and only got myself to complete it now - yes, got the toy.

Description

Something dark has taken root within you, <name>. The tendrils of N'Zoth are burrowing into your mind and soul, eager to consume everything you are. I can perform a rite of cleansing, but only in a place that resonates with the very power that grips you. Meet me near the Crucible of Storms. The ritual will be perilous and painful, but it is your only chance to be free. The Old God has eyes everywhere. I do not wish you to become one of them.
