There's no way to avoid the throw that guarantees you lose 30 (or 150 sanity with mask) besides beating him fast. In an 8-mask run you may have to save a midcombat orb just to make him beatable.
As solo tank he threw me 3 times (aka 450 of my 550 sanity) by 45% hp.

This guy is hardly unbeatable, but does feel a little cheap compared to most mini-bosses in Horrific Visions.

All other bosses have Sanity draining abilities that are telegraphed and avoidable, or interruptible. The fact that you can't prevent this guy's toss ability going through at all is somewhat frustrating, especially when trying to do high Mask runs. That ability smacks you for -150 Sanity, when your maximum is less than 600 with all upgrades. It's brutal, and I'd even say it's borderline unfair.

I highly recommend saving this area for last, because you will almost certainly need to burn an Orb during combat just to survive this guy. No matter how fast you try to burn him down, you're gonna eat at least one of his tosses.

For what it's worth, with the mask of Multitudes, you don't have to kill any of the side bosses. Just avoid this guy. The quest in that zone is percentage base and you fill it really fast with just the trash.

Edit: apparently not. Not sure what I did on my first clear earlier but i did not have to kill him.

So after some testing.. im not sure if thats how supposed to work or not but if u keep moving into the hitbox of the mob like literally inside of him he sometimes doesnt throw you, and proceeds to the jump instead, its tricky to execute .. but thats the only way i was able to avoid his throw

no clue if intended design, but it seems like he's no longer using the sanity drain leap if you tank him close to wall - i think preferably with your back against the wall

still leaps though, which eats sanity too but is avoidable

How to avoid the "unavoidable" sanity drain

I did this with a warrior but in theory the other abilities mentioned here should work to prevent the throw or keep you from losing sanity to it. Keep in mind that the next throw may come before your ability is off CD though, so get him down quickly. Feel free to reply to this comment if you try with an ability and confirm it does or does not work.

Warrior - Heroic Leap after he throws you but before you land (I’m going to try charge next time to see if that also works but I’m guessing it won’t have enough range until it’s too late). Bladestorm would also work if you have it up when he’s throwing you.

Death Knight - Death’s advance

Paladin - Bubblehearth (get out of here, cheater)

Hunter - Disengage

Shaman - Gust of Wind

Monk - Transcendence

Druid - Wild Charge (may require a specific form, I don’t know. I’m not a druid.)

Demon Hunter - Fel Rush/DH Jump thing (maybe even just glide? unsure)

Rogue - Shadowstep maybe?

For pet classes - if you stand far away from him, he wont attack you / drain sanity at all. He did absolutely no sanity damage or otherwise to me on 8masks just because i stayed as far away from him as possible while still being able to deal dmg. Good luck!
