There are 4 different types of Malignant Hearts, Vicious Hearts (Offensive), Brutal Hearts (Defensive), Devious Hearts (Utility), and Wrathful Hearts ("Super Power").

While the first 3 types (Vicious, Brutal, Devious) of hearts can be farmed and crafted somewhat reliably, obtaining the powerful Wrathful Hearts is a bit more complicated.

Malignant Sockets in Jewelry have a color corresponding to the Malignant Heart type: Orange for Vicious Hearts, Purple for Brutal Hearts, and Pink for Devious Hearts.

Wrathful Hearts can be socketed into all Malignant Sockets.

Malignant Hearts have different levels - the higher the level, the higher the affixes that come with the Malignant Heart bonus.

To counteract the fact that you aren't able to socket Gems into Jewelry anymore, all Malignant Hearts will provide an Armor bonus which scales with their Item Power.

How Malignant Tunnels Work

The best way to farm Wrathful Hearts and other Malignant Hearts is to farm Malignant Tunnels, the new seasonal mini-dungeons all over Sanctuary.

Malignant Tunnels will contain a few Malignant Elites and an end-of-dungeon event called Invoke the Malignant.

To start this event, you will have to use a so-called Malignant Invoker, an item players can craft at Cormond's Workbench in all major cities.

You will have two options to activate the Malignant Outgrowth and to start the event: You can either use a Wrathful Malignant Invoker, or either a Devious, Brutal, or Vicious Malignant Invoker.

The second choice (Devious, Brutal, Vicious) will always be displayed when you hover over the dungeon tooltip on your World Map - that way you can target-farm one of these three Malignant Heart types.

How To Farm Wrathful Hearts

There are two ways to farm Wrathful Hearts: You either run Malignant Tunnels over and over until you find a Malignant Elite with the Wrathful affix, or you craft a Wrathful Malignant Invoker and use it at the end of a Malignant Tunnel during the Invoke the Malignant event.

Wrathful Malignant Elites are rare spawns in Malignant Tunnels, which makes sense due to the sheer power level of Wrathful Hearts.

During the Seasonal Questline, you will get one guaranteed Wrathful Heart drop: Varshan's Heart. This item will have either the or the Wrathful Heart bonus.

It looks like defeating Echoes of Varshan will also be a somewhat reliable way to farm Wrathful Hearts.

Malignant and Wrathful Invokers

You can craft Brutal Malignant Invokers, Devious Malignant Invokers, and Vicious Malignant Invokers with Brutal Malignant Ichors, Devious Malignant Ichors, and Vicious Malignant Ichors.

You cannot craft Wrathful Malignant Invokers - these are only contained in the randomized Uncertain Invoker chests which you can also craft with Malignant Ichors.

You can reliably farm Brutal, Devious, and Vicious Malignant Hearts by crafting invokers and using them to activate the Invoke the Malignant event at the end of a Malignant Tunnel.

How to Craft And Use Invoker of Varshan

At the end of the Seasonal Questline, you will get Recipe: Invoker of Varshan. The end boss you need to kill as the final quest step will also drop an Invoker of Varshan.

Invoker of Varshan is used to spawn the Echo of Varshan, a low-level version of the new Seasonal Uber Boss.

To use the Invoker of Varshan in World Tier 2, you will need to find a boss chamber in a Malignant Tunnel which will contain the Echo of Varshan event. It is important to note that this has to happen in World Tier 2!

Killing the Echo of Varshan in World Tier 2 will reward great loot, a decent chance for a Wrathful Heart drop, and a Foul Invoker of Varshan (as well as the Recipe: Foul Invoker of Varshan) which can then be used to summon the Echo of Varshan in World Tier 3.

Defeating Echo of Varshan in World Tier 3 and 4 happens in the same way - you need to kill the World Tier 2 version to get access to the World Tier 3 Echo of Varshan fight, and so on.

Malignant Hearts Tips and Tricks

You can only equip 1 Wrathful Heart at a time. You can equip multiple Vicious, Brutal, or Devious Hearts.

Once you have killed an Elite monster with the Malignant affix (Brutal, Devious, Vicious, or Wrathful), you'll need to wait a few seconds for the interactable Malignant Heart item and the "Malignant version" of the Elite to spawn. The second version of the Elite then will drop a Caged Heart after it has been killed.

Caged Hearts on the ground are indicated by a small leaf icon on your Mini Map and World Map.

Be quick - the interactable Malignant Heart item that spawns the Malignant Elite can despawn after about 20 seconds!
