Flapping your wings while dragonriding seems to proc this trinket.

Literally everything seems to trigger this thing, time to make a list of actions that do it.

Flapping your wings.
Summoning a mount.
Mend pet tick.
Profession knowledge point items.
Changing tracking.
Talented exhilaration tick.
Certain enchantment that heals you out of combat.
Opening this one bag with crafting reagent.
Killing insects while doing some random world quest.
Something something this talent, yeah.
Editing this comment, most likely.
Using insignias to raise your rep.
Crafting stuff.
Jumping on your divine horse.
Filling a vial with random and totally safe fluids found in a sealed vault.
Throwing and then using a positional displacer (which is probably totally safe and all, after all those years) you found in the same vault.
Getting some of that knowledge points you work hard for every week.

Edit (06/12/2022): Changing equipment seems to do it as well.
Edit (12/2/2023): And I'm back to tell yall that even the mend pet tick seems to trigger the effect yo, so does using a certain herbalism item. Hunter tracking also seems to do the deed.
Edit (14/2/2023): Regenerative leech out of combat regen ticks do seem to be a trigger for this thing as well.
Edit (8/3/2023): Opened account bound satchel with primals, got healed.
Edit (10/3/2023): Been doing world quests, bam the trinket worked it's magic yet again.
Edit (17/4/2023): Word on the street is that this talent also seems to proc this.
Edit (22/4/2023): Seems like you can get a proc when crafting or using an insignia to raise your dragonflight renown reps.

Edit (26/4/2023): Did some profession quests, got some profession knowledge usable items, bam it happened again.

Looks like this got nerfed in the 1/10/23 patch? Tooltip now indicates additional 4,997 damage and 7,514 healing. Used to say 15,753 damage and healing, which admittedly did seem a bit much for a lvl 327 item. (No wonder AMR wouldn't let me part with it. Guess that'll change.)

I've had some great trinkets over the years. This is one of them. Super useful.

Make sure if you're a hunter to put this in your bags this when casting Beast Lore. Had a couple of target pets get stung hard and die by this. I thought my Beast Lore button was broken.
