I was very disappointed to find out this doesn't actually grant X mastery per Neltharite per stack, it actually grants X mastery per stack and you get one stack per Neltharite. The result is there really isn't any point to stacking the gems, at best you're just cycling faster and the payoff upon hitting 15 stacks is tiny.

The way this works:
You equip it, you go and hit the target dummy, at some point it procs and grants you a number of stacks with infinite duration. The number of those stacks is equal to your neltharite gem total across all your equiped items. For me its 5, so i start hitting the dummy and the first time it procs it grants me 5 stacks, which at 418 ilvl is 53x5=265 mastery. I now have +265 mastery with infinite duration. I keep hitting the dummy, and at some point it procs again and takes me to 10 stacks, so 530 mastery. Later on it procs again and takes me to 15 stacks which is 795 mastery. At this point, when it procs again, it consumes all stacks and grants me a buff named Gift of the Aspects which increases all my secondary stats by 229 for 15 seconds. After those 15 seconds, it is back to step 1 with 0 stacks.

So, this trinket works in a "cycle" similar to the mage talent Incanter's flow. It adds an "ebb and flow" kind of thing to your damage, which is entirely out of your control and not worth playing around. I cannot think of any spec in the game that would benefit from holding its cds to line up with the stronger phases of this cycle, as each second your cds are ready and you're not using them is lost dps which outweights the benefit by far. As such, you can treat this trinket as a Whispering Incarnate Icon, eg an entirely passive stat stick that sometimes gives extra benefits but you shouldn't bother doing any mini-games for it.

Only thing worth noting is that if you get to 6 total neltharites across your gear the cycle will end up being 0 -> 6 -> 12 -> Gift of the Aspects, so the trinket will probably lose a bunch of value because its strongest phases are when you have 10+ stacks or the gift. This can be simmed, and if it ends up being a big enough loss you can play around it by socketing a non-neltharite gem in your 6th slot, as this trinket only cares about your neltharites.

All in all this is a good stat stick if you were unlucky with your Furious Ragefeather drops, for my own character Ruskbreath-Silvermoon the idol at 418 sims just a tiny bit higher than a 415 ragefeather. Totally not worth losing your mind over how and when it procs, just craft it and slap it on if you were unlucky with your trinket drops, as you would any other piece of gear.

the tooltip on this trinket isn't the same in game, there's a 10 stat difference for the same ilevel (405 in game has 50 mastery, on wowhead it has 40).

Why does this thing SIM so well? I do have 5 neltharites which seems to be the sweet spot but it sims 1500+ dps better than 411 manic grief torch.

I dont want to waste a CPI on it when i could have chest or main hand crafted instead? both the chest and main hand sim any where near this trinket... can someone allay my confusion :)
