Overview of the Metamorphosis Runeword
Metamorphosis is a Druid helm aimed at Shapeshifter druids. While Blizzard clearly wanted to encourage players to switch between the two main forms (Wolf and Bear), for all practical purposes, this Runeword supports fighting primarily as a Wolf and using the Bear form as a temporary buff.
By switching between Bear and Wolf within the allotted 180-second durations, Druids can have major buffs that improve both their survivability and DPS output through major bonuses to attack speed, attack rating, maximum life, and reduction in Physical damage. Outside of the buffs, it also offers Crushing Blow, as well as some not-terribly-impressive stat bonuses. Cannot Be Frozen is nice but there are usually more than enough ways to get this on other items.

Metamorphosis may be greatly improved by changes introduced for the beginning of Season 4 to shapeshifting. Prior to this, changing from Wolf to Bear forms and back again required shifting through Human, with a 1-second delay between shifts (Wolf > Human > Bear > Human > Wolf etc). Now, you can shift directly between Wolf and Bear without having to travel through Human form. There's still a 1-second delay between shifts, but at least you don't need to go through the vulnerable Human stage. This will make this much more viable and powerful going forward.
Metamorphosis Recommendations
Runewords vary on their suitability for different classes and specs, as well as utility for the various Mercenaries. This next section will discuss what classes and specs can make the most out of Metamorphosis, whether you should consider using Metamorphosis with your Mercenary follower, what base items to consider, and where to acquire Runes.
Best Classes and Specs for the Metamorphosis Runeword
The best build to go along with Metamorphosis is a Fury Werewolf build, rather than a Maul Werebear build. The extra speed you get synergies much more effectively with the speed you already get from Fury, making you a blur as you rip through your enemies.
It also serves to shore up one of the biggest issues with a Werewolf build: its relative squishiness compared to other melee builds/classes. You'll still have to deal with Physical immunes and Bone Break still has challenges to keep it on all the time (namely, the issue with increasing Physical damage you take, which this build cannot offset entirely).

Since there are no shapeshifting druid Mercenaries (indeed, no Druid Mercenaries of any kind), Metamorphosis is not a viable Mercenary item.

Elite Druid Pelts (Blood Spirit, Sun Spirit, Earth Spirit, Sky Spirit, Dream Spirit)
There is absolutely no good reason to use anything other than a Druid Pelt with extra skills on board for Metamorphosis. It would be leaving power on the table to use anything else. There isn't a great deal of point spread among the various elite quality Druid Pelts; these range from a minimum of 98-109 to a maximum of 147-159. Get whatever one gives you the very best skills natively. All Druid Pelts can have up to 3 Sockets, so there's no worries there. Since Metamorphosis can't even be made until level 67 due to the Cham rune, you will be able to use any of these. The highest, Dream Spirit, has a minimum level of 66.
