Tier 16 is a 5-piece set obtained with tokens from 决战奥格瑞玛.
There are three different colors: 528 gear from LFR, 540 from Flex shares colors with 553 from Normal, and 566 from Heroics. This is the normal/flex color.
These tokens are turned into tier from vendors at your faction's shrine:
Alliance: 克拉丽丝·查普曼 (Normal), 德里特·岩心 (Flex)
Horde: 奇阿戈努 (Normal), 纳迪娜·星晶 (Flex)

Tier drops are as follows:

Helm: Normal and flex tokens from 嗜血的索克 (Wing 3) in 决战奥格瑞玛.
Shoulders: Normal and flex tokens from 攻城匠师黑索 (Wing 4) in 决战奥格瑞玛.
Chest: Normal and flex tokens from 傲之煞 (Wing 1) in 决战奥格瑞玛.
Gloves: Normal and flex tokens from 纳兹戈林将军 (Wing 2) in 决战奥格瑞玛.
Legs: Normal and flex tokens from Klaxxi Paragons (Wing 4) in 决战奥格瑞玛.

In addition, 加尔鲁什·地狱咆哮 drops a special tier token that can be redeemed for any slot, like 受诅征服者精华.
The four world bosses for 天神挑战者 drop ilvl 553 gloves and legs, and 斡耳朵斯 drops matching ilvl 559 non-tier.

Matching accessories:

Waist: 斡耳朵斯 or 碎地者哈洛姆
Feet: Fallen Protectors, 解锁的潘达利亚战利品.

Would have been nice to have more variance in the boots of the set, they're all the same model. Personally I look for the bulky type like 静息之靴, so it's a shame to see only that.
