I stepped on all the runes, nothing happened. I got bored of waiting so I started killing off his minions, and then he spawned. Given the fact that his spawn timer seems extremely long compared to all other rares, any combination of this might be the catalyst.

I tried running through the runes, he spawned when I reached third rune. Killed few minions aswell.

Didn't step on runes, didn't kill the small skeleton adds.

Waited patiently, and after about 5-10 min he spawned.
