I'm sorry, but this is simply the ugliest off-hand item I've seen in a very, very, very long time. I simply don't understand how this passed any finalization checks at Blizzard HQ. It looks like a giant baby rattler with spasmodic blue spagetti all over it.

It seems that this is the only off-hand you can get from the higher-level T14 content. You can get it from the second boss (刀锋领主塔亚克) of the first wing of 恐惧之心, called 恐惧临近 in your LFR dropdown. There isn't a spirit off-hand at this ilvl, and it's possible this is only available to damage-based specs (this is what atlasloot claims, but I'm not sure where it got that information). Because 长者的好运符 are now pretty easy to get, you can just re-run this wing and use your bonus roll until it (hopefully) drops.

The next off-hands you can get will be 九头蛇神像 from 火焰之头 in the second wing of 雷电王座, listed as 被遗忘的深渊 in your LFR dropdown, and 电弧宝珠 (铁穹) or 雷神的命令宝珠 (雷神), both from the last wing of ToT, listed as 风暴之巅.

You can also get 风火扇 from MSV, or find 耳语秘言之卷 as a rare drop from trash mobs (I picked one up doing Shado-Pan dailies), and 铭文翠扇 can be made by scribes, but all are ilvl 476, compared to this item's 483 ilvl.

恶毒角斗士的缓刑 and 恶毒角斗士的决战 are now available for honor, but are both only ilvl 476. Previously the stat allocation was pretty bad for PvE, but after 5.3 these are actually viable. I had also previously thought there was an 7200 season honor requirement for weapons and off-hands, but it seems that's been lifted.

暴虐角斗士的决战 and 暴虐角斗士的缓刑 are available for conquest, and are ilvl 498. Previously the stat allocation was bad for PvE like the honor off-hands, but after 5.3 these are actually now better than the normal mode 唤风香炉 (an extra 19 stam, 12 int, and 63 secondary stats), and healers can pick the spirit version. They require 1250 conquest points, which you can grind out in one sitting if you're good/lucky enough, and do not appear to have a minimum season conquest requirement. If you're bad at or don't like PvP, you can also grind the conquest from the Isle of Thunder PvP dailies at 50 conquest per day, but it will take 25 days. Still, I'm at 950 conquest and haven't seen an off-hand from ToT, so if you don't mind the dailies, this is a viable source of off-hands now.

Note that the PvP gear cannot be upgraded, while the PvE gear can, so you're definitely better off with the 502 off-hands from ToT than the conquest off-hands, but the conquest off-hands have a guaranteed "drop rate", while the LFR gear does not.
