These are indeed dropped by Majordomo Staghelm. Assuming these are like the other tier shoulder tokens that have dropped in the past, this particular item will have about a 35 - 40% drop rate on 10 man, and a 70-75% drop rate on 25 man.

It looks like as of 6.0 this can't be traded in anymore. Now you just pay gold for the piece of gear that used to be used with this item.
