Do these guys even exist anymore? I've been searching for 2 hours and have only noticed "Intrepid Ghouls". Are they only available during certain current quests?

Yes Spiked Ghoul exist and if you passed that phase it's OK. I'm short on quest so I gave it a shot. I went to 32.7, 64.4 and cast mirror image then used my Crashin' Thrashin' Racer Controller to pull him and when he was near enough my mirror images started attacking him. I stopped controlling my racer and finished him off. Make sure you are in Icecrown and not The Fleshwerks or Spiked Ghoul will not appear. NOTE: Kill it in Icecrown or you will not be able to loot it. Good Luck!

if you not have done the quests of Darkrider Arly

then the quest item WILL NOT DROP after you dont her quests

and you have get Spill Their Blood then the ghoul start drop

DROP IT WITH each time you kill one spike one
