TLDR:
This is a melee range attack on the 1st use, then it switches to a ranged attack, and the 3rd use is the first two attacks combined.

When you use this trinket the first time, it does a 'sweeping' slice type attack in front of the player at melee range.
The melee attack looks like a purple shadowy crescent in front of the player.
When you use this trinket the 2nd time, it shoots out 3 blades of ice straight ahead of the player the direction they are facing. The ranged attack looks a lot like Jaina's 3 ice blade attack she uses in the End Time dungeon.
When you use the trinket the 3rd and final time, it combines the two attacks.
None of the uses requires a target.

Tooltips for the 2 extra attacks:

2nd Use: Open the pouch and draw Vx's frost blade to launch a long range attack, inflicting 371,256 Frost damage split between enemies it passes through.

3rd Use: Draw the blades once more to perform both strikes as the same time.
