You get this mount as a kyrian via path of ascention covenant feature.
To get this mount just upgrade your Path of Ascention to rank 2 and defeat Mad Mortimer on Loyalty (2nd difficulty) difficulty. :)

I did this using Pelagos and used the following charms and equipment:

Charm of Fortitude
Charm of Persistence
Charm of Quickness
Phial of Serenity

Keep in mind on this fight, all slimes share health pool! The charms might be more than necessary, but I wanted to have breathing room.

When you first fight the red slime, you can line of sight it behind a pillar. When the red slime has come all the way to the pillar you don't have to worry about it touching you as it will always stop moving and start casting once it sees you. At this point if you use Confront Memories and touch the spell haste memory, you can usually get 2 shots of Test of Faith per his one cast. Keep doing this until you can use Unleash. If you don't feel comfortable doing the double Test of Faiths, just pick a different memory, or none if you want to save energy, and whittle it down until the green slime appears.

When the green slime appears you mostly do the same thing except now you have to be aware of the path the green slime is going to charge forward too. After he finishes his charge he will erupt slime puddles on the floor for you to have to avoid from now on. All I did here was avoid green slime guy and continue focusing the red one since you have to line of sight his cast. I tried to make use of one pillar as much as I could so the green slime was mostly contained in one area until I had to go to another pillar. Make sure you're using your phial of serenity regularly as it's on a 45 second CD and you most likely have taken some splash damage by now.

When the blue slime appears, he walks around dropping ice puddles. Things start to get hectic with 2 different sets of puddles. At this point I had to kite among 3 pillars in an almost triangle fashion. You may want to use the speed and/or shield memories depending on whether you're having issues kiting or needing to walk through lots of stuff on the floor or worried the red one will get a cast off while you run between pillars to avoid the other 2 slimes. Just remember that it's always more important to snare the blue, then green, then red slime since the blue one is the only one that continually walks towards you. While you kite you can try to maneuver them to walk near each other and snare them in a group, but you don't have to make that your main goal.

TL;DR
- Red slime casts when in sight. Just line of sight it before the end of its cast and get your casts off.
- Green slime does avoidable straight line charge and erupts goo at the end. Try to use one area of arena at all times until you need cleaner ground. Focus either slime with attacks and line of sight red slime cast.
- Blue slime walks towards you and drops ice puddles. Make him your snare priority. Keep an eye on green slime charge path and don't be in it. Keep an eye on red slime cast bar and line of sight it. Attack any slime that is most convenient for you to keep up this kiting and line of sight method.
