Got this item recently and the info on it and where to get it is few and far between, so hear is what I found:

1. It can drop off the WQ rare naga listed here. The drop rate is miserably low. Between alts, I had well over 40 tries and no luck. I did see it drop for party members very rarely. I may have even seen it drop twice on the same kill. If your RNG is good or you are really in no hurry to get it, keep grinding these guys and eventually it may drop. Not for me though.

2. Rumors circulated that it could be in treasure chests found while using scrying stones. I have never seen anyone confirm this. I personally opened no less then 400 of these treasure chests with no luck. I suggest you get your 100 for the achieve and stop there.

3. The only other possibility I found in random comments here and there was from a specific Arcane Trunk needed for the Trove Tracker Achievement. This was the only one I was missing for that achievement and the zone meta. And with good reason, it is the easily the hardest of the 8 Arcane trunk puzzles. It is located at 61,23 (entrance 61,20). There are tons of posts that this is bugged/glitched/too hard and unbeatable with any lag. When I beat it, I was running 14fps so don't believe them.

The order of the orbs is always the same. There are 2-3 posts listing it and I pasted it on my screen for reference. The orbs come too fast at the end, so I never actually referenced the order. Sound, which I thought would help at first, because there is definitely a rhythm in the early stages turned out to be unnecessary. This is straight up "Simon Says". Keep doing it until you memorize the order of the first 4-5 volleys without doing unnecessary damage from misses. Keep trying until you are flawless for the first half. Towards the end, the orbs go so fast you are almost mashing the keyboard. Note that you can press any/all of the 3 buttons at the same time. It gets hectic, but perfection in the first half makes up for mistakes in the end. Beat the puzzle, opened up the chest and got this totem.

People have down voted someone listing this same spot. I can't vouch that it is 100% drop rate, but many posters say they have gotten it from this specific chest. I highly recommend trying this spot if you are looking for this item. You probably want the zone meta anyway, so put in the time to beat this instead of wasting hours and hours chasing the lower RNG sources.
