Got one from Oubdob da Smasher, but nothing from the other world quest enemies. Killed him two times, but only got one. Anyone else gotten one from someone?

One just dropped for me, looks to be from Warden kills:

Reputation with The Wardens increased by 250.
Reputation with Dreamweavers increased by 250.
DANGER: Malisandra completed.
Received 402 Gold, 15 Silver.

EDIT: After a few more Warden kills it looks to be just a chance to drop like others have suggested
EDIT #2: Managed to get 4 drop yesterday, could be due to the fact I have been away for a few days due to hurricane. I am currently 8/13.

SO! In order to get 1 Skull you must (A) Have a wardens DANGER! quest. (B) It must be a humanoid like creature. and (C) It is not a 100% drop chance. OH! and you must also be artifact lvl 5 or higher. so you cant start the second you hit friendly with all the races. so the long and short of it is screw affliction warlocks. there are 3 freaking layers of RNG to get around. all of which must happen in order with no deviation. and you also need 13 of the buggers. This is the worst way to get the skin. Its downright unfair when some classes can just go grind mobs to pick it up or even buy the damn thing.

I think they should make the drop chance 100% and keep the other two requirements. Being humanoid and the Danger mission. or maybe even let it drop off of last bosses on heroic or mythic/mythic+. idk it seams ridiculous to me to have to be gated by so many factors i cant even affect to improve my odds.

DO NOT! I repeat do not kill the any of the humanoids in the Danger world quest in a Raid. It will not drop skull. Such a waste of time.

On 12/13. The last weekend got 2 humanoid bosses but strangely neither dropped a skull for me. Yesterday got one from Captain Dargun in Stormheim. (Which makes me 12/13)

No humanoid danger quest today damn it! Gotta wait another several days! I'm totally not mad :)

To clarify, the skulls only drop from DANGER world quests with human-like targets (those marked with a dragon border on the world map). They can drop from Humanoids, Demons and Giants, provided they are roughly shaped like a humanoid. For example it can drop from Felguards, but not from Felbats.

It does not drop from WANTED quests at all.

ok quick question, do the mobs spawn and drop skulls when the quests are not running? Or do we HAVE to wait for the quests to be available to be able to kill them and loot the skulls?

Just wanted to put in an updated post for any late blooming warlocks with names and locations.
I spent quite a while digging through comments on here to compile the list of potential skull droppers.

Just to reiterate what others have pointed out:
1.) You do not need to be in affliction spec to get the quest starting item or to get skulls
2.) You cannot get the skull item if you are in a raid group, only 5 man or solo.
3.) Just because a name is on this list, the skull isn't a 100% drop (as far as i know). Some report getting a skull from certain mobs and while others did not.
4.) I am posting this in such a way that you can copy/paste into a macro for TomTom. If you used Macro Toolkit addon and extend the macro character limit then it should all fit in two macros (divide it out however you want, i've sorted my macros by zone)... otherwise you will need about 4 individual regular macros.

Azuna zone, 危险：卡萨克斯 drops this questitem. It is not looted from boss via loot window, but is automatically added to your quest counter.

Mob is a demon, standing on a ledge just by the water over a small cave.

Just a heads up, you can apparently get them from Wanted: quests now that have humanoid type mobs. I tried a giant and did not so may not be 100% but I definitely got one from a Wanted quest earlier.

So, to consolidate and list various.

This drops from DANGER Warden world quests. It does NOT drop from WANTED Warden world quests. It also requires a reasonably humanoid target, be that an undead, a rokbar, giant or similar. Thus, quests like 危险：巢母伊尔瓦 will not net you a skull. You can NOT be in a raid group and get the drop. Any more than 5 members and you're out of luck. Also, while the drop rate is HIGH...it isn't 100%, so don't get too upset if you don't get a drop from a mob that should give one.
Quests known to give this include,

I did some testing and you only get the skull upon completing the actual WQ. Killing the mob the next reset while the WQ is still active but already completed does not reward a 2nd skull.

Just Curious. Do you have to have the Affliction Artifact equipped? Killed Fjorlag, the Grave's Chill today while in Demonology spec and no skull. Not in raid/group.

I have been able to obtain said skulls from both WANTED! and DANGER! world boss world quests. Maybe they have updated it since 2017. No need for Affliction spec or have the artifact weapon equipped to obtain them. Hope this helps.

Just to confirm. Ive tried multiple "WANTED" quests targeting humanoids and have not had the skull drop. It appears to drop only from
"DANGER" quests
