If you can't get it from the vision, you can probably get it for Ysera herself in Nordrassil. That's what I did. Hope this helps.

You can not pick this quest up if you have Wings over Mt Hyjal. Drop that quest as its only worth 2k xp and this one is worth 13k

I first completed Wings over Hyjal and after completing Return from the Firelands she gave me this quest

I've done all the previous quest's and also cant get it.
It's not possible to get it in Nordrassil

There are two Vision of Ysera with different IDs listed in the quick fact. I'm not sure which one materializes at Shrine of Goldrinn (28.4, 29.8). I'll need to do the previous quest again on another alt to find the correct ID, because she's faded on me.
Edit: Vision of Ysera appears at Sanctuary of Malorne (27.0, 62.7).
Also, Ysera isn't listed in the quick fact. You can obtain this from her, as pointed out in previous comments.

In case anybody else is flying up and down the tree, she's on the main floor all the way in the back. (Ignore the ramp; the shrine is "deeper" than similar structures.)

Locate Choluna at the Shrine of Aviana in Mount Hyjal.

Description

Your expertise is needed elsewhere, <name>.

Just southwest of the World Tree you will find the Shrine of Aviana, home to the Druids of the Talon. Aviana, the matron of all winged creatures, gave her life for this world ten thousand years ago.

I sense her shrine is being threatened, as are the druids who guard it. Seek out Choluna, one of the wisest of the winged druids, deep within the shrine.
