Just like the quest says, he's overlooking the Watch from up there.

If you head west you will see a ledge with cultist members. Go toward the back of this area and there is another platform where the Black Knight is encased in a bubble with cultists surrounding it...the doctor walks around on this platform.

I think this guy's name is a possible reference to Dr. Kohler from Dan Brown's book "Angels and Demons," who I suppose also had the personality of an old, undead grouch. =P

It is important to note that unlike Deathspeaker Kharos, Doctor Kohler does not give credit towards Taking The Battle To The Enemy when killed (presumably because he is not a member of the Cult of the Damned).

I'm not sure when this changed, but now he drops between about 35s - 1g50s in loot.

It used to be his corpse was lootable, but nothing would be there (unless you had the quest, then he'd drop the quest item.)

It's a good idea to kill this guy while doing your Argent Tournament dailies as he drops an average of 1 gold each time. I seen it as low as 40s but also as high as 1g50s.
