You have to realize that
Tears of Blood
can only proc from critical heals but this trinket can be on as long as the person is healing.

Tyrande's favorite doll would average out to about 350 MP/5, I'm more interested in Jar of Ancient Remedies (basically, 79% up-time 515 spirit, and then a use of 6420 mana every 2 minutes). That would average out to 203 MP/5 from spirit and 267.5 for the proc, total of 470.5.
For healing, I'd rate Jar of Ancient Remedies > DMC: Tsunami > Core of Ripeness > Blood of Isiset (Heroic) > Tyrande's Favorite Doll > Sea Star (Heroic). It's a steep curve in effectiveness for each of them, too.

Because that would be really important for Resto Druids.
And what about the Proc chance of the initial buff, and then the proc chance to get more stacks?

Inscription at 525 skill can make these cards. They're known as Darkmoon Cards of Destruction.
Keep in mind however that when a scribe creates a card, it is completely random and can be any card (from Ace to 8) from any of the new decks (waves, stones, winds, etc) so it may take a lot of crafting for a single scribe to render a deck. Might be better to get any card you can and trade around for what you want or if you have the cash just buy them from the AH.

Just got this as a resto druid, it does NOT proc from casting hots, neither from hot ticks for me. The only instant casts that trigger it are swiftmend and NS HT..
And it seems to have a 10sec internal cd. I doubt it will me up all the time in fights.

Ok got this trinket, it does have a 10 sec ICD before another stack can be applied. As a holy priest however POH and COH give it an instant 5 stacks.
Holy priest-
4016 intellect
3050 spirit
combat regen 3938.
Equiped
+120 combat regen
when at 5 stacks, I receive +397 mp5
Replenishment the intellect gives /2= 24 mp5
So at full stacks I get 120+397+24= 541 combat regen bonus from this trinket.

is better may not have calculated holy priest and the latest patch to Holy concentration. 4200 mana every minute divided by 12(12 being combat regen in a minute) you get 350 mp5 not adding the intellect replenishment.
I for one am very happy to have landed this, paid a pretty penny but well worth it. I should also get better regen with kings up as well, not a huge gain but more is more. 541 is a bit better than 350.
I'd love to do some math on Jar of Ancient Remedies, +515 spirit for a holy priest would something be along the lines of 480-500 mp5 depending on intellect scaling. DMC just might end up ahead and gives a small boost to Spell power, and SF/arcane torent.

For paladins, this trinket will grant you 2 stacks if you have beacon of light cast on a target in your party or raid.
One for your spell, and one for the beacon heal.
After using it some more, I've also noticed that the beacon heal seems to ignore the internal cooldown, so a paladin can stack this up by just chain casting to get beacon of light heals.

After further testing its a smart trinket do this and you will see make a /cancelaura giant wave macro and spam a 2 second heal 30 times whell clicking macro inbetween it will proc 100% after sit there and throw 1 heal every 12 seconds you will stack to 5 and refresh without every losing a stack. My results seem to point to me that this is a "smart" trinket that has a set proc rate but checks if you have buff on you and under X seconds has a 100% proc rate I believe it to be between 8-10 seconds left as the magic number. This also points to people getting different results in there testing for proc rates.
edit upon further testing maybe its just 100% proc rate when casting a true 3 second cast timer spell upon further testing.

For some wierd reason the 10sec internal cd only applies when in combat, i tried the trinket in an heroic yesterday and it only takes like 5/6 seconds to stack it up depending on how fast i can push out the heals

Light of dawn works to stack the buff with this trinket. In heroics I just LoD my group right before pulls and it puts up 5 stacks immediately which stay up until the fight is over. No worrying about silly proc rates and icd's.

Instant 5 stacks when using Healing Rain (as well as most AOE skills from other healers, as mentioned in other comments).
Also worth mentioning that, as of time posting, this trinket will boost the spirit from Mana Tide (as does Jar of Ancient Remedies).

I got this trinket tonight.
It does not seem to proc off Rejuvination, Lifebloom, or Wild Growth, instead preferring direct heals. The larger druid heals seem to refresh the stacks 100% of the time though, so you can bubble it up to 400% and keep it there using Swiftmend, as if you needed yet another reason to use Swiftmend.
EDIT:
This no longer has an internal cooldown and procs off any heal, not just a spell, including hots... It goes to 5 stacks in 5 seconds with 1 lifebloom now.

From a holy pally POV: This trinket is completely OP and worth every copper. The 321 Intellect buffs us in so many ways it's ridiculous, and no one can argue with 400 Spirit. :)

Click on Buffs Gained or Buffs Cast (either works). You'll see that Giant Wave procced for me an astounding 5,699 times during the course of four hours of raiding, compared to my resto druid friend (click on the buff Giant Wave) who only had 405 recorded procs.
The comparison is complicated by:
1) He was boomkin for Magmaw, meaning he probably had a different trinket; and
2) I was out of range of him for a large portion of Conclave of Wind, so it's possible the WoL didn't catch the trinket procs during the Conclave encounter.
If those numbers are correct that's about 14x the number of procs the druid got. Even if the numbers are inaccurate, it's very obvious that this trinket benefits some classes more than others.

It can proc 1 stack from Holy Shock, Holy Light, Flash of Light, Divine Light, Word of Glory.
It double dips with Beacon of Light. You get one proc from the initial heal and one proc from the Beacon heal.
It can proc all 5 stacks on one GCD of Light of Dawn if you're lucky and aim it right.
It does not proc from Holy Radiance.

All in all, it's extremely difficult *NOT* to keep this up as a holy paladin during normal raiding, between the length of the buff and the number of heals that can add one or more stacks. TL;DR: If you're a holy paladin, get it, love it, profit!
Edit: The ICD appears to be much shorter than 10 seconds or even 8 seconds. Going from the above WOL I was able to pull this:

I have not observed any internal GCD on this. It seems to proc with back to back Nourish's or Regrowth. Not 100% of the time, but often enough.

Anecdotal evidence, but I can confirm that on a resto druid, the trinket currently
does not proc
on Rejuvenation, Lifebloom or Wild Growth.
Procs quite happily
on Healing Touch, Nourish and Regrowth; seems to proc 100% of the time on Swiftmend. So resto druids in a raid situation should have no problem keeping the stacks up, as Swiftmend's on a 15 second (at most) cooldown, and the stacks last 20 seconds.
If/when Blizzard fix the trinket so that it procs on HoTs too, we'll be able to get the stacks up even faster, so there'll be no issues at all.
On a side note, for those who are having trouble getting hold of the rarer cards for this deck, don't forget - there's always the other faction's auction house! I ended up getting my Four on the Alliance side.
For those who aren't familiar with cross-faction trading, here's a brief guide:
1. To get the gold across factions: On your cross-faction character, put a junk item on the neutral (Booty Bay / Tanaris) auction house, for the amount of money you want (plus 15-20%, to account for the neutral AH cut.)
2. Swap back to your home faction and buy the item. Wait one hour.
3. Collect gold on cross-faction character. Purchase item from cross-faction auction house.
4. With the help of a home-faction friend (preferably using vent, or Real-ID), put the item on the neutral AH and have your friend purchase it immediately.
5. Trade with friend, create deck, profit.

This trinket is amazing for Holy priests, even more so with the upcoming buff to Holy Concentration. I just purchased this card earlier to day and started messing around with it.
This card is probably the easiest stacking item in the game to keep 100% uptime for all 5 stacks. For Holy priests, the trinket procs off EVERY heal in one way or another. It does not proc off HoTs like Renew, BUT it does proc off Echo of Light(Mastery) HoT. I was able to Renew 1 person and have 5 stacks in roughly 2-3 seconds and as long as I have a Renew on someone, all 5 stacks stay up 100% of the time. So basically, if you cast Renew atleast once every 17-18 seconds, this card is a free 400 spirit.
Other AoE/HoT spells that proc this card:
CoH; 5 stacks
Prayer of Healing; 5 stacks
Renew; 5 stacks in roughly 2-3 seconds from Echo of Light
Holy Word; Sanctuary; Almost immediate 5 stacks with 1 person standing in it, any more than 1 person and it is instant.
Holy Nova; No immediate stacks from Holy Nova, but it does stack up quickly from Echo of Light.
Binding Heal; 2 stacks
In addition to these are Heal, Greater Heal and Flash Heal, which all provide 1 stack for the immediate direct heal and additional stacks from Echo of Light.
All in all, this card is amazing for Holy priests.

As of 1/6/2011 this trinket does
now
correctly stack with Heal over Time spells.
It was a surprising and very pleasant change that I noticed the first trash pull into Bastion. With only 2 stacks of Lifebloom applied, I had a full 5 stacks of the trinket rolling, being continuously refreshed by my HoTs.
Very happy Resto Druid here.

My main is an 85 holy priest.
I dont think they will "fix" the not proc on HoT thing as it would be way too easy to keep the proc stacked up. As a raid healer i often have renew ticking on every player in the raid or most of them. It ticks every few seconds. That means it would be impossible not to keep it stacked up 100% if you had a heal like renew. Dont hold your breathe for them to make it work on hots.If they make it work on HoTs they may as well just make the trinket +321 Intel +400 spirit and be done with it.
That being said its a nice trinket for Holy Priest.....over 4k mana pool, a few % crit, about 150 mp5 from the intel and 400 mp5 from proc if you keep it up with GH or COH every so often. But im not paying the 30K+ it goes for on my realm right now. 359 gear is going to be like lvl 200 gear was in WOTLK. Good for now but superceded not too far in the future. Then you would be "gee i wish i didnt spend the 30k on that". lol.

On average for a decently geared Holy Priest, you are looking at roughly .25-.3 of the intellect converting to combat regen, strictly from the regen formula. For most healers though the intellect bonus is negligible as they already have a trinket or 2 with the +321 int on it already.
If you assume a 100% uptime (or close to it) of the full 5 stacks of the spirit buff, you end up looking at +400 spirit obviously. However, 400 spirit for holy priest doesn't directly follow to combat regen. If talented and increased slightly because of higher intellect values (~4500-5000) the spirit to combat MP5 ratio is roughly .92. For the Jar, an average time between casts was taken and the mana gained summed for each interval of each stack amount, up to 5 where it is assumed 5 stacks remain until the on use is used, then holding at 0 stacks for the 30 second cooldown. For static use trinkets the duration divided by the cooldown is the ratio used for average spirit.
MP5 from +/- INT by 321 = ~ +/-90

It is worth noting that the most stable trinkets are Fall of Mortality, Jar of Ancient Remedies, Darkmoon Card: Tsunami, and Shard of Woe. The others rely on a potion or an action to receive it's full effectiveness, which sometimes isn't activated every time the cooldown allows. Vibrant Alchemists stone is good if you need the secondary Haste, but for any endgame fight over 4 minutes, which there are a lot, it loses a lot of value, and is only good if a potion is used during the fight which isn't always necessary.

This is wonderful for pretty much all healing classes but being a bit biased I'd have to say it's super good for holy pallies. In an expansion where overhealing with direct heals is no longer an issue, the 321 spellpower and near 4k+ mana is just too good to pass up, not to mention the mp5.
However, if you're like me, and a WoW welfare case, you might also look at Fall of Mortality, which, while not as good as this in the long run, is still a pretty nice chunk of mana return with the added 321 intellect. Currently using Tyrande's Fav. Doll and Fall of Mortality and loving them both. If this card happened to fall in my lap though I'd replace fall of mortality with it. (Depsite the coolness of the cho'gall trinket. =-p)

I used this trinket in my heroic Ragnaros kill, we used 3 healers in our 25 man, and this trinket continues to be a holy priest #1 trinket. You get 721stats from this trinket at full stacks, 100% uptime. How can you go wrong.

Still using DMC:T on Dragon Soul as a Holy Paladin. After 4.3 mana regeneration changes for holy paladin , i think it is time to look for an upgrade which has more intellect.
