Note that this quest comes from the end of a long chain asking you to go everywhere and do everything. Always.
It starts with a Battered Hilt, which is a drop off of mobs in either of the Icecrown Citadel 5-man instances.

You don't need to have ever been to the Sunwell Plateau raid.

You will end up in a 'phased' version of the instance, and will only be able to walk around in a limited region of it: the hallway running up to the actual Sunwell, which is now* a place of pilgrimage for Blood Elfs. You won't need to fight anything.

*: only for the people being at this specific part of this quest chain...

You do not need to be in a raid to do this portion of the quest. Once you have accepted the quest, talk to any of the NPCs around the quest giver to be teleported into the Sunwell Plateau raid instance. When you are inside, you should see the Sunwell, down at the bottom of the spiral slope (you really can't miss it). Just go down, and click your tainted blade next to the Sunwell. It will kind of suspend there in mid-air.

At this point, the two NPCs will start a dialogue, and at the end of the dialogue, you can click on your suspended sword to pick it up again. There's actually a port to Dalaran available at the side of the circular area, so don't bother hearthing. You do, however, need to fly back to the Argent Tournament area to turn in the quest before coming back to Dalaran again, though.

The phasing of this portion of the quest is very interesting, as most of the isle has been taken back. Except the naga along the coast line, who were left to torment the murlocs there.

This seems to have different scenarios for different races. I just completed this on my blood elf hunter who was welcomed to the Sunwell and cheered as he purified the blade, and hailed as the one who would avenge the fallen of Quel'thalas.

Guildie of mine just finished it on her troll hunter and as soon as she got close to the Sunwell she was frozen in place and Loth'remar tried to claim the blade from her, but was unable to as the blade chose her.

Interesting differentiation of events.

There is a bug where upon entering the instance via the quest npc you can't get to the sunwell because a gate is blocking the way.
Leave the instance, set it to 10 man raid, reset it and re enter. this fixed it for me.
