You have to fly to the Wyrmrest Temple (or use Dalaran HS and the portal straight to the destination point if lazy to fly).

Trizormu can be found at the first floor of Wyrmrest Temple.
Tariolstrasz stands near the west entrance of WT, but he will throw you out as you asked him to help.

Fine, fly down to the underground sanctum (there's a shatter through the ground north of the WT), Ruby Sanctum entrance is left as you entered to the hall.
Codex Draconomicus placed on a table at the south part of Ruby Sanctum, try dont pull that lazy wyrm in the middle.

This quest is quite frankly unacceptable. The dragons are now infertile and these are the last ones there will ever be, so the DK goes waltzing in here completely unprovoked and start slaughtering them. DKs are unequivocally the bad guys in this situation, there is no other way it can possibly be interpreted. And it makes me angry at Blizz for constantly crapping on dragons, because nothing good has happened to them at all since Cata ended: They turn infertile, the black dragons are almost all killed, the blue dragons disband and abandon the Nexus, the bronze dragons have to rely on mortal Timewalkers and Kairoz betrayed them, Ysera and a bunch of other green dragons are corrupted and die, and now the last holdout for the reds - the very protectors of life itself! - is slaughtered unprovoked.

I'm sick and tired of it. It really feels like Blizzard just hates dragons.

It would be so cool if there were actual different endings whether you kill the dragons or not.

Encountered a bug where this quest was not automatically given to me after seeing the Frozen Memories scene in Icecrown. If this happens to you, there is an NPC in Acherus named Slimy who will have this quest available.

I am not able to read the codex before killing all the dragons, and still not able to after killing all the dragons.

There does not appear to be any special interaction if you kill *none* of the dragons. I did try. For posterity. Now to kill them all, I guess.

The Feat of Strength for killing all of the red dragons in the Sanctum has been removed for the live version.

So if you just committed a massacre and are here looking for word on why you got no achievement, that is the reason. In the words of Blizzard:

"Feedback from the community noted that a missable achievement was unfun for the players that missed it. While we've chosen to remove the Feats of Strength granted in the questlines overall, the voice line from Bolvar will still play if players do choose to go down this path as Death Knights.

As was alluded to, these Feats were not meant to go live, as we had previously decided that they were misaligned with our general look towards achievements, namely that they were unobtainable if missed in a non-repeatable quest area. Again, these will no longer be obtainable in 7.2.5, and for players who did obtain the achievement it will be removed."

And for those that had it removed:
"I can appreciate that no-one likes having something taken away from them. To try and clarify a little bit why we came to this decision; during the course of the PTR testing feedback from the community overwhelmingly pointed out that a missable achievement was both unfun for players that missed it, and unfun for players that felt compelled to slaughter innocent red dragons to achieve it.

I do also want to mention that, while the Feat of Strength will be removed, the special voice line from Bolvar will still play if players choose to follow this particular route."

When I tried to complete this quest, I cleared my way to the book, but it wouldn't let me interact with it. I tried /reload, logging out, turning off all addons, and dropping the quest and getting it again, but none of those would work for me. I then tried doing "Reset all instances" and that worked for me. It brought back all the dragons I killed and I had to clear a path again, but I was finally able to interact with the book.

While it might go against what our characters stand for as death knights, it IS actually possible to do (for lack of a better term) a pacifist run of this quest. If you're a Night Elf DK, you can Shadowmeld and do this easily, but for everyone else:

As soon as you enter, go to the right path as it has less mobs as you head for the ridge in the back where the book is, going between the huge dragon(Vyrstrazsa) in the middle of the room and the Ruby Keepers to the side. Stepping on the red flowers in the middle won't aggro the big one or any of her whelps. Hide behind the huge trees if a Ruby Warden patrols by until you get to the ridge.

Now for the hard part. There are two packs of 3 mobs on either side that you *cannot* avoid aggroing and 1 mob, the Ruby Lorekeeper, right next to the book on the table. You need to read the book, which takes about 1 second, to gain credit for completion, but you get interrupted if you get attacked.
Hug the hill to your left as much as you can, use your mount to immedietly rush to the table (aggroing the 3 mob pack in the process), use Strangulate on the lorekeeper to stun her, and you SHOULD have enough time to read the book without being interrupted. As you finish, simply use Wraith Walk to get the hell out of dodge before the dragonspawn kill you.

No tangible benefits, but a fun little challenge nonetheless.

Trizormu says: Ack! Alright! There used to be... glaciers... large ones... to the north of Icecrown... they floated away years ago... I don't know any more, I swear! Speak to... Tariol!

Tariolstrasz is less helpful:

Your actions stretch the limits of our alliance, death knight.

Tell me all you know of a great dragon that fell on a glacier to the north of Icecrown.

Tariolstrasz says: You will never find what you seek, death-bringer. Now, you have worn out your welcome at this temple, begone!

Seeing as how buggy this quest is, I'd recommend doing this step by step.
Open your quest log and see what you have to do, make sure every step is completed before proceeding.

Edit: when you enter the ruby sanctum, make sure a dialog pops up near the entrance and that you have received quest credit, otherwise you will have to run out and reset instance to interact with the book again.

My DK is exalted with The Wyrmrest Accord so the guards greeted me with "Dragonfriend" after having strangled Trizormu!

The Feat of Strength for killing all of the dragons has been removed from the game. You can just run to the book, kill minimal mobs and get the job done much faster now. Don't bother killing anything you don't have too.

The quest has autostart. Some addon will block the autostart and you might miss the quest.

If you abandoned this quest or forgot to start, or were not able to start it in the previous instanced area, simply return back to your class order hall "Archerus" and talk to Slimy. He will offer the quest for you.

For those idiots like me who haven't played in ages, fly around the ruby sanctum and you'll find a tunnel under it. Don't try to make snow angels or cast Death & Decay on the snow patch outside, non distructible terrain :/

Interestingly the music seems to change to Icecrown music if you choose to kill all the dragons.

As of the point of writing the achievement seems to be gone but is replaced with a short voice clip from The Lich King saying "You are empty inside, just like me."
