On a standard Windows keyboard I also pulled the left Winkey off a long time ago. However, with my Microsoft Natural Elite ergonomic keyboard (seen in the picture) certain keys are extended for ease of use. This makes pressing the Winkey during a game I am familiar with a non-issue. Not to mention the ease of typing, which I do a lot of, hehe.
WASD to move, 1-5 w/ shift for my DPS spells. Holy fire, SW:P, MF, etc. I have a macro bound to my mousewheel (mmb), so in combination with ctrl/shift/alt I have access to at least 6 more spells: Fade, PW:S (self, shift), levitate (ctrl), PS (ctrl+shift), shadowform (alt+shift), PoH (ctrl+alt). I then use Clique for all of my healing: GH on mouse 1, PW:S on mouse 2, renew on mouse 3, and GS on shift+m2, along with various permutations of ctrl/shift/alt to get dispel, flash heal, PoM, CoH, levitate, etc.On my rogue, I have a "branching" macro bound to mmb, so it changes which macro is pressed depending on if I'm in combat, stealthed, in combat and stealthed, etc. Makes great use of the /click command. I plan on extending this concept to my priest, to give me a different set of macros for whether I'm shadow or holy spec.I also have the \ keys bound for frequent utility usage, things that don't need to be seen. don't function while flying.\ is mounts: black bear (nomod), grand mammoth (shift), zebra(ctrl), and TLPD (alt)] is inner fire[ is fun stuff: Mind soothe (nomod), mind vision (shift), levitate (ctrl)

You can press the Alt with your thumb, allowing you to push everything you can with your left hand if you are a typist.
I generally use Shift + 1-6 for changing hotbars, too. However, what I discovered was that for me I would find it more efficient to use mouse 4 and mouse 5 or the mouse wheel to do that. What about using the mouse to zoom in or out? I set up several preset camera angles beforehand. This can be done through through the Set camera angle key binding section in Options.
Speaking of keybinds:Is there anyone else that use ESDF for movement? I find that very handy when it comes to keybinds. By moving the movement scheme a button to the right opens up for more keybinds on the left, leaving more utility to the pinky and allows you to reach more buttons on the right. Afterall, the hand rests at the same keys as you would when you type in touch.And while we're at it, does someone remember where this scheme comes from? Must've been Gothic 2 or Morrowind, but I can't recall.
Why not just use SPACE as a key modifier. And map jump to a different key :D.Its way easier to reach than other key modifiers and your hand position stays the same.
It probably would have been more helpful if I explained I use ASDF for movement instead of WADS, but even when using WADS it's easier--for me--to hit the CTRL key with my hand while pushing SHIFT with my pinkie. So there's a use for it regardless of your key setup. Of course, it depends on personal preference.
I've been using shift, alt, and control since I found out how to bind skills to keys. I use 1-7, q, e, f, z, and tilde for common bindings. Alt+whatever is used for pets, control+whatever are trap functions, and shift+whatever are other skills (MD, FD, Kill Shot, etc) that I have no room for on my bar and don't need to use readily.I also use alt+shift+1-3 for raptor strike, mongoose bite, and my disengage macro*. * - Disengage + Wing Clip
