Mine is:
When i started my DK at the launch of WotLK, i used paladin tank gear with spellpower and stamina.
Just because i though it would make my AoE damage rise alot.
You werent the only one. I saw plenty of DKs wearing spellpower plate(still see some till this day) back when WOTLK launched. Blizz needs to really specify what stats certain classes need in the character selection screen so this type of stuff doesnt happen.
As for me, it was leveling my shaman resto when i was new to the game. God it took so long until someone told me i should spec enhancement for leveling. After that leveling was a breeze and i never made the mistake of leveling while specced as a healer.

When I hit 70 on my Warrior, he was in a 20/20/21 spec. He was the very first character I rolled and I didn't know anything about specs, so I was just sticking points anywhere that looked cool. I figured out before I hit max level that this was incorrect, but I decided to wait until 70 to respec.
I didn't pick up dual wield on my Enhancement Shaman until after I hit Outland, since I was afraid the loss of a shield would lower my survivability. And when 3.0.2 hit I just recreated the spec I had been using--with points in Ele and Enh--without going all the way down the Enhance tree to pick up my new 51-point talent, not to mention the awesome talent that precedes it. That mistake I fixed immediately after starting to quest in Northrend.

When I first started playing my paladin, I had no idea you were supposed to pick one talent tree and fill it up. When I was <40 I had points all over the place. I got a cool looking shield around 40-ish and someone from my guild gently suggested I might spec fully into the protection tree to get the shield abilities. I did that, and wow, it worked. Heh.
One time with my hunter, I was in VOA and we cleared the first 2 bosses, I turned on aspect of the pack to run down to the last boss, and forgot to turn it off! :O
Yes, I was *that* huntar in the raid. I felt like sinking right through the floor when I realized. Thankfully the raid were all well overgeared for it, and they were laughing about how that was one way to make the last boss of VOA an actual challenge.
I have not forgotten to check my aspect since then either!

Back during the whole 'Distracting Shot' change, almost wiped my raid like 4 times before I read the new tool tip and learned it's just a taunt for the hunters now instead of allowing extra threat to be added on during MD.

Last night, i was in a pug doing 10VoA on my mage, doing koralon, when i had flames shot at me, so to get out of them, i decided to use Blink to get out of them (which i usually don't do), this caused me to land directly under Koralon when he was about to use Meteor Smash, I luckily got away from it. But it's probably my biggest/stupidest mistake for me in a raid.
Or the time that i was in a Guild run of Naxx on Heigan and i had forgotten that as a ranged class i was supposed to run right past him, on to his platform. So i did phase 1 of the dance as a ranged class blasting him away with Fireblasts, Icelances and CoC's

A few times while healing on my priest, I've had myself targeted, wondering why the tank's health wasn't going up. Hasn't resulted in a wipe just yet... but I imagine it will some day.

Levitate/Slow Fall glyphs are the greatest thing added to the game.

Jumped off of a cliff after rearranging buttons on my action bars, realizing that I hadn't re-mapped levitate to any of the bars after I was already airborne. At least I was riding my
Spectral Tiger
at the time so I at least died in style.

My first toon in vanilla was a druid. I came to WoW with maybe 2 weeks of EQ experience as far as MMO's go.
So I had talent points in every tree and i wore any piece of gear that was... blue. Yes, blue. Blue was special. Whether it had SP, Def, STR or Agi, it was blue and beautiful.
Thankfully, I didn't play for long and only came back towards the end of TBC. By then, I had gotten into a good guild and my brother started playing again a few weeks later. He directed me to sites like Wowhead and EJ to better learn my class.
A lot of time later... I now have 5 80's (druid, DK, mage, lock and pally)
My two worst moments I have never forgotten and both shortly after those toons had reached 80 and i was still learning my role in raids:
On my DK (unholy)
After a wipe on Patchwerk in Naxx25, I raised my ghoul... too close to Patch, aggroed and wiped the group again. It was so bad, the raid was called T_T I was so embarrassed (it was a pug, so a lot of flaming and omg, deathtard! comments) that I didn't play her for 3 weeks.
On my mage:
After a wipe on Thaddeus, all of the pats were not cleared before the room he was in. I was running back, and someone said to be careful of the mobs. I hit invisibility... but i hit it too early and it wore off just as i was running under one of those big giant pats. I died and it went after the few other people who had just run back themselves. They were upset, but thankfully nice enough about it that the raid wasn't called and I have never used Invisibility for that again ^^;
